Ultrasound diagnosis of impingement syndrome of the shoulder joint

Summary. The work deals with investigation of ultrasound signsof impingement syndrome of theshoulder joint. Thefindings of ultrasound investigation of shoulder joints of 62 patients with complaints of a prolonged (over 1 year) pain in the shoulder area, limited movement in the joint without obvious trauma in the history were analyzed. The ultrasound findings were compared with those of MRI and radiography. Ultrasound signs of impingement synd- rome, i.e. thinning of the rotatory cuff, sharp increase of its echogenicity, calcifications and partial impairments, limited and almost completely lost movements at functional load, were revealed. Ultrasound investigation is non-invasive, accessible and highly effective method of diagnosis of impingement syndrome of the shoulder joint, with sensitivity exceeding that of MRI due to its capability to perform a functional test and possibility to assess vascularization.

The role of Doppler measurement in the the adducting uterine veins in diagnosis of endometrial tumors

Summary. The work deals with investigation of the capabilities of transvaginal ultrasound investigation in combination with color, energetic and spectral Doppler investigation in differential diag- nosis of benign endometrial hyperplasia and cancer (EC). The work is based on complex investigation of 66 pre- and post- menopause patients with historically verified pathology of the endometrium. It was revealed that in EC the thickness of the endometrium is greater than in benign hyperplasia, the outlines are uneven, indistinct, the structure is more inhomogeneous, vascularization is greater, peripheral resistance index is lower. Transvaginal ultrasound investigation improves the efficacy of the diagnosis of endometrial cancer which can be used as a highly effective method at early stages of the disease.

Interrelation between atherosclerotic involvement of carotid arteries and development of ischemic stroke

Summary. The work deals with investigation of interrelation between the character of atherosclerotic involvement of carotid arteries and development of ischemic stroke using Doppler ultra- sound findings. The work is based on the findings of ultrasound intracranial Doppler investigation. Different echographic variants of atherosclerotic plaques of the carotid arteries are shown. Their embologenicity was investigated. The findings of the research prove that triplex sonography is a highly informative method of diagnosis of the atherosclerotic involvement of carotid and cerebral arteries. Ischemic stroke can be caused both by a critical stenosis and occlusion of carotid arteries as well as moderately pronounced stenosis.

Optimizing the protocols of neoadjuvant therapy for operable rectal

Summary. A method of combination neoadjuvant therapy for operable rectal cancer consisting in pre-operative large-fraction intensive irradiation against a background of endolymphatic chemotherapy with 5-fluorouracil followed by surgery within the period of 72 hours was worked out at Donetsk Regional Antitumor Center. This method does not influence the frequency of intra- and post-operative complications and results in increase of the 5-year survival.

The experience of combined treatment for cancer of the head and neck skin at Ivano-Frankivsk regional cancer hospital

Summary. The results of combined treatment for skin cancer located on the head and neck at Ivano-Frankivsk regional cancer hospital are analyzed. It is shown that pre-operative radiotherapy (close-focus x-ray therapy) allows to improve the efficacy and quality of treatment in this group of patients as well as to prevent the cases of the disease prolongation and relapses.

Intra-operative catheterization of the hepatic artery at colorectal cancer with liver metastases

Summary. A method of regional intrahepatic chemotherapy in the liver of the patients with inoperable malignant tumors of the liver was worked out and introduced to the practice. The efficacy of the method was demonstrated in treatment of 65 patients with primary and metastatic cancer of the liver and gastrointestinal tract. Locoregional chemotherapy allowed to achieve the optimum concentration of the drug in the tumor focus, improve the drug efficacy with sharp reduction of toxic side effects.

Individual radiation sensitivity of the tumors and its purposeful modification (radiobiologic and clinical aspects)

Summary. The influenceof different factors of formation of individual tumor radiosensitivity, which should be considered with the purpose of its modification, was investigated. The findings of the original radiobiological and clinical investigations of the use of radiotherapy adjuvant (hyperthermia) at combination radiation therapy for uterine body cancer are presented.

The experience of brachytherapy application in combination radiation therapy for malignant tumors of the oral and oropharyngeal mucosa

Summary. A technique of combination radiation therapy for oral and oropharyngeal cancer was tested. Distance radiation therapy was performed using POKYC-M or ATAT-P1 units, brachytherapy was delivered using ATAT-Byi unit. In each case application acrylic phantoms were made. This allowed to perform brachytherapy in patients with poorly accessible tumors, improve the results of treatment and quality of life as well as to prolong the life of the patients by 1-3 years.

Contemporary approaches to conservative treatment for local breast cancer

Summary. Effective methods of conservative treatment for local breast cancer (BC) at application of a complex of cytostatic methods are shown. One hundred and three patients with BC were treated. The patients of group 1 (36 persons) were administered chemo- radiation therapy (DRT and two sessions of PCT using modified FAC protocol). In the patients from group 2 (35 persons), DRT was administered against a background of cytostatic drug Ftorafurin a radio modifying dose. The controls (32 patients) were administered only radiation therapy. Significant increase of the tumor regression in patients with local BC at CRT and radiation therapy against a background of Ftorafur administration was noted.

Toxicity of conservative treatment for local cervical cancer and its prevention

Summary. The authors analyze the toxic manifestations of chemoradiation therapy (CRT) against a background of chemoradiomodif ication and apoptosis inductors in 76 patients with local cervical cancer (LCC). The investigation performed at clinical radiooncology department of National Cancer Institute suggests than individual approach to the choice of CRT technique with the consideration of the findings of complex investigation as wel 1 as a thorough pre-radiation preparation, individual computer planning, accurate delivery of the planned radiation therapy and prevention of toxic complications and therapeutic programs promote impotent of efficacy of conservative treatment for LCC.

The prospects of improving conservative treatment for generalized cervical cancer

Summary. Radiation Oncology Department of National Institute of Cancer has developed and improved methods of conservative treatment for local forms of cervical cancer (LCC) based on the use of radiomodulating properties of cytostatics of f luorpyri- midin group (o-fluoruracil, Xeloda/Ental, Phthorafurum), hydroxiurea and interferon (IVF). The indications and contra- indications to their use were determined. Increase of the degree and rates of the tumor regression were observed in patients with LCC who received combination therapy together with fluorpyrimidin drugs at radiomodif y ing doses and IFR. The analysis of the obtained findings showed that individual approach to conservative for LCC and timely correction of the treatment program allowed to improve the treatment efficacy and did not result in its toxicity.

Interrelation of proteolysis reactions, antiendotixin immunity and apoptosis markers in the blood serum at radiation therapy with radiomodification

Summary. The investigation of 18 patients with head and neck tumors demonstrated that radiation therapy with radiomodification produced more intensive normalization of proteolysis parameters, increase of blood serum TNF-a level and reduction of the indices of humoral and antiendotoxin immunity, when compared with the group of the patients without modification. Normalizing proteolysis parameters and TNF-a level increase can be considered as one of the possible criteria of efficacy of RT with radiomodi- fication, which requires further investigation.

Contemporary approaches to radiation and accompanying therapy for intracerebral malignant gliomas

Summary. The analysis of the results of complex treatment of 212 patients with intracerebral gliomas and astrocytomas allowed to suggest an effective protocol, including radical surgery, modified hyperfractionated radiotherapy, the use of chemotherapy (Temodal, Xeloda) as radiomodifiers as well as target therapy (Avastin) with monoclonal antibodies. Mean survival median of the patients with giomas at the above approach to the treatment makes 3.5 years, those with astrocyto- mas — 4 years.

Treatment for inoperable forms of cervical cancer

Summary. The technique of chemoradiation therapy for advanced forms of cervical cancer (CC) is featured. The performed investigation allowed to prove that the use of chemotherapy with antibacterial therapy before multimodality therapy in patients with primarily inoperable CC promoted improvement of immediate treatment results as well as reduced thenumberof post-radiation complications, which improved the quality and duration of life of the patients.

The peculiarities of spermatogenesis disorders in patients with testicular seminoma after unilateral orchidectomy and radiation therapy

Summary. The changes in the spermogram were investigated in 57 of 145 patients with stage I-IIA testicular seminoma after unilateral orchidectomy and radiation therapy to the para-aortic region. Preventive irradiation of the iliac and supraclavicular lymph nodes was not performed. In 86 % of the patients pronounced inhibition of speramtogenesis with its restoration 36 months later were registered. Observation tactics is promising in patients with stage I testicul ar seminoma after unilateral orchidectomy.

An experience of phototherapy application to prevention of early skin radiation reactions in patients with breast cancer

Summary. Theinfluenceof phototherapy with red ( 630- 660 nm) and blue ( 440-470 nm) light on the skin of the patients with breast cancer during the course of post-operative radiation therapy was investigated. It was established that phototherapy positively influenced the state of the skin in the area of the irradiation; red light promoted chiefly prevention of early skin radiation reactions, while blue light considerably improved their course. Phototherapy also positively influenced the state of the blood in patients during radiation therapy increasing hemoglobin level and leukocyte amount.

Secondary malignancies in patients with stage IA-IIIA Hodgkin's lymphoma after radiation (chemoradiation) therapy using accelerated dose fractionation

Summary. The incidence of secondary malignancies was investigated in 367 patients with stage IA-IIIAHodgkin's lymp- homa after radiation (chemoradiation) therapy using accelerated fractionation. For 20 years (median 14.5 years) of the observation 24 of them developed 27(7.4%) tumors, besides their frequency did not depend on the disease stage and method of treatment.

Up-to-date methods of robotic surgical treatment with a cyberknife system (CyberKnife G4)

Summary. Robotic radiosurgery system cyberknifeis used to treatcancerpathologyof the whole body as well as neoplasiaof the central nervous system with the methods of single-fraction radiosurgery and radiosurgery in 2-5 fractions with 6 MeV photons. The world experience and the original observations suggest a high efficacy and safety of this method of radiosurgery.

Assessment of the quality of life of the patients with stage III non-small-cell lung cancer at radiotherapy with nonstandard fractionation modes

Summary. A positive effect of radiation therapy (RT) for non- small-cell lung cancer (NSCLC) was produced by accelerated modes, which were accompanied by improvement of the quality of life of the patients in all groups due to positive changes of the clinical signs. When compared with the standard mode, accelerated irradiation produces a clinical effect 2-3 weeks earlier. Radiation pulmonitis and fibrosis of the lung tissue negatively influence the quality of life of the patients with NSCLC after radiation therapy within the period of 3-8 months of follow-up, which necessitates accompanying therapy during the radiation therapy and the follow-up.

Combination of radiation therapy and endolymphatic chemotherapy in palliative treatment for liver metastases

Summary. The variants of multimodality palliative treatment for liver metastases are featured. It was established that irradiation of the liver area using the suggested technique did not produce radiation reactions and complications associated with radiation lesion of the organ. Combination of endoymphatic chemotherapy with radiation therapy is more effective when compared with systemic polychemotherapy.

The experience of GenferonR application in patients with cervical cancer against a background of multimodality therapy

Summary. Tolerance and efficacy of Genferon® as an accompanying therapy in patients with cervical cancer during the stages of multimodality therapy and rehabilitation were assessed. Suppositories Genferon® were administered to 28 women during multimodality therapy and for 3 months afterit. After the treatment, vaginal administration continued every other day at a dose of 500 000 IU for a month, 2 times a week for 3 months with the purpose to prevent adhesive colpitis. A tendency to increased rate and degree of the tumor resorption as well as significant reduction of the incidence of early and late local radiation reactions, improvementof thequalityof lifeduring the treatmemt and rehabilitation was noted as the result of the treatment. Side effects were not observed.

Analysis of long-term results of multimodality and surgical treatment for gastric cancer

Summary. The experience of treatment of 318 gastric cancer (GC) cases, of them 178 (56%) were administered multimodality treatment (pre-operative radiation therapy followed by surgery) and 140 (44%) only surgery, is reported. The analysis of the long-term treatment results showed that at early pTl GC combination treatment did not have any advantages over surgery; both methods were characterized byhighsurvivalrateindices. In patients with involvement of all layers of the wall and presence of metastases to regional nodes (T3N1-2), multimodality treatment with pre- operative radiation therapy has statistically significant advantages (p < 0.05).

Neoadjuvant selective intra-arterial tumor chemoembolization as a radiomodifier at combination treatment for laryngeal cancer

Summary. The work deals with investigation of immediate results of treatment of patients with laryngeal cancer who were administered radiation therapy in combination with neoadjuvant selective intra-arterial tumor chemoembolization. The work is based on the results of treatment of 103 patients. The use of the suggested method improves the efficacy of treatment due to reduction of manifestations of late radiation lesions. Chemoembolization produces not only radiosensibilizing effect in the tumor but also protects relatively healthy tissues.

Influence of Spirulina on the state of spleen proteolysis in irradiated rats

Summary. The mechanisms of spleen intracellular proteolysis function in rats after fractionated x-ray exposure at a dose of 25 cGy as well as correction of the damages to lysosomal-vacuole apparatus of the cell with Spirulina drug (Spirulina platensis) are featured. It was established that spleen proteolysis system in all investigated terms was destabilized and had a tendency to activation.

The ways of improvement of combination therapy results in patients with local cervical cancer

Summary. A new solution of a scientific task of modern oncogynecology, improvement of the efficacy of treatment for local cervical cancer (CC) on the account of expansion of the indications to operative treatment is presented on the clinical material (275 patients with stage II-III CC). The use of the developed technique of multimodality therapy based on the split course of combination radiation therapy against a background of neoadjuvant chemo- therapy allowed to convert in 49.6% of cases of immobile tumor process to an operable stage followed by uterus and adnexae removal while at the traditional combination radiotherapy theresectability index was 6.9%. The developed method of chemoradiation therapy for inoperable local CC allows to reduce 1.5 times the frequency of early radiation complications when compared with the controls as well as completely exclude such late radiation complications as fistula formation. Satisfactory results of 5-year survival, 68.4% forthe general group and 75.5% for the operated patients were obtained.

Left ventricle structure and function assessment using multispiral CT ventriculography following myocardial infarction

Objective: To determine the degree of myocardium lesion in patients after myocardial infarction (MI) and reveal its complications using multispiral computed tomography (MSCT).
Material and Methods: The investigation involved 61 patents (57 men and 4 women) aged 33-72 with clinically confirmed diagnosis of myocardial infarction and suspec ted early and late post- infarction complications. MSCTcoronarography/ventriculography was performed using 16, 32, 64-slicecomputer tomography units, Light Speed 16, Light Speed Pro 32, Light Speed VCT 64 with the data processing with cardiology workstations Advantage Workstation 4.2, 4.3 and 4.4.
Results: The capabilities of MSCT in diagnosis of myocardium ischemic lesions in MI and revealing MI complications using MSCT- ventriculography were assessed. The investigation revealed increase in LV volume parameters and reduction of ejection fraction as well as disorders of the LV myocardium contractility in 29 patients (47.5 %). This group was mainly composed of the patients who survived large-focus MI. MSCT- ventriculography revealed LV aneurysms in 36 patients, of them 2 acute and 34 chronic aneurysms, and demonstrated their location, size and shape. Thrombotic complications, rupture of the inter- ventricular septum and other extracardiac MI complications were also revealed.
Conclusion: MSCT-ventriculography with contrast enhancement allows to reveal the defects of the myocardium contrast in patients with acute and subacute MI and determine the location and degree of myocardium ischemic lesion. When MI complications are sus- pected, MSCT aids in confirming or exclusion of post-infarction complications, which can be important for further treatment of the patients with myocardial infarction.
