The influence of the stage of the disease and degree of the tumor regression at radiation therapy on survival of the patients with inoperable epidermoid lung cancer

Summary. Main prerequisite of relatively high quality parameters of survival at radiation therapy for epidermoid lung cancer is complete tumor regression. One-year oncospecific survival of patients with stages I+II at complete tumor regression makes 86.4 ±4.2 %, with stage III — 86.3 ± 4.8 %; when compete effect was not achieved — 49.8 ± 3.9 % and 43.5 ± 4.1 %, respectively. Three-year survival of the patients with stages I+II at complete regression makes 40.2 ± 6.1 %, with stage III — 37.4 ± 6.9 % ; when compete effect was not achieved — 9.3 ± 2.3 % and 4.5 ± 1.9 %, respectively. Five year survival of the patients with stages I + II at complete regression makes — 19.7 ± 5.2%, with stage III — 13.3 ± 5.2 %; when compete effect was not achieved — 6.6 ± 2.1 % and 0.0 + 0.6 %, respectively.

Tumor marker CYFRA 21-1 in assessment of radiation treatment efficacy at local non-small-cell lung cancer

Summary. The results of radiation therapy (RT) at various fractionation modes with Etopiside and Cysplatin modification in patients with non-small-cell lung cancer (NSCLC) are ana- lyzed. The objective effect of accelerated RT was accompanied by statistically significant reduction of blood serum CYFRA 21-1 level (an index of proteolytic tumor degradation). It was established that the dynamics of blood serum CYFRA 21-1 expression coincided with the clinical findings and could be used for prognosis and monitoring of chemoradiation therapy efficacy in patients with NSCLC.

Hematological toxicity of neoadjuvant chemoradiation therapy for gastric cancer

Summary. It is shown that the developed by the authors and applied in proactive method of neoadjuvant chemoradiation therapy allowing improvement of longterm results in patient with gastric cancer did not produce a marked toxic effect on the patients and did not increase the frequency of postradiation complications.

Radiation therapy for operable rectal cancer

Summary. The authors present a review of the literature on modern tendencies of radiation therapy application to treatment of operable rectal cancer. Many randomized control studies compared the efficacy of combination of radiation therapy (pre-operative or post-operative) and surgery versus surgery only demonstrating various results. Meta-analysis of the data on efficacy of combination of radiation therapy and standard surgery revealed 22 randomized control studies (14 with pre-operative radiation therapy and 8 with post-operative radiation therapy) with total number of 8507 patients (Colorectal Cancer Collaborative Group, 2000). The use of combination treatment reduced the number of isolated locoregional relapses both with pre-operative (22.5 - 12.5 %; p < 0.00001) and post-operative radiation therapy (25.8 - 16.7 %; p - 0.00001). The influence on total survival was not significant (62 % vs. 63 %; p - 0.06).

Combination radiation and intra-arterial therapy of patients with colorectal cancer with mechanical jaundice

Summary. The article features a variant of combination treatment for colorectal cancer with metastases to the liver against a background of mechanical jaundice using intraarterial PCT followed by a course irradiation of the liver region by the original techniques. The efficacy of the suggested technique vs. the early existing ones was proven.

I. The variability of individual kinetics of aberration yield

Summary. The cytogenetic damage was measured in blood lymphocytes of mammary cancer patients in the individual follow-up during their radiotherapy course. For the first time the intraand inter-individual variability of the kinetics of chromosome aberration accumulation was quantified for radical and adjuvant radiotherapy progammes.

II. The effect of chemotherapy

Summary. The presence of the first-line chemotherapy in the breast cancer treatment scheme was associated with the moderate increase of the cytogenetic damage yield in patients' blood lymphocytes and also with reduced intensity of chromosome aberration accumulation during the radical radiotherapy progamme.

Cytogenetic lesions at radiation induced occupational lung cancer in uranium miners

Summary. The examination of the group of uranium miners with occupational lung cancer and miners without cancer showed an increased level of radiation induced disorders of the chromosomal apparatus of the cells when compared with the cytogenetic indices of the controls. Reduction in the level of cytogenetic disorders was not observed when the work at the mine was discontinued.

Dosimetry of stereotaxic patient irradiation

Summary. A list of the tests for quality assurance of an accelerator necessary to perform stereotaxic irradiation of the patients is given. The tests are described briefly, their frequency is given. A technique of assessment of beam axis targeting accuracy at the isocenter of the accelerator, developed at N.N. Alexandrov National Cancer Center, is described.

Promotion of high-quality treatment and uninterrupted service of radiation therapy linear accelerators

Summary. The experience of operation of radiotherapy complex at Donetsk Antitumor Center showed that a number of calibration and dosimetry tests were an inevitable part of high-quality radiation treatment and timely prevention of inconsiderable disorders in the work of linear accelerators, therefore the engineering service of this equipment should consist of various specialists.

Pre-operative radiochemotherapy for low local rectal cancer

Summary. The results of neoadjuvant combination radiation therapy were analyzed in the group of patients with inoperable T3-T4N0-N1M0 rectal cancer depending on various chemoradiosensibilizers used during the irradiation. The best tumor response and achievement of the tumor resectability were observed in case of irradiation sensibilization with Capecitabine 1500 mg/m! per day.

Approaches to chemoradiation therapy of patients with relapses and metastases of brain cancer

Summary. A possibility and efficacy of chemoradiation therapy (CRT) in patients with relapses and metastases of breast cancer (BC) with the history of radical treatment (combination or chemoradiation therapy with hormone therapy) for local BC is shown. The treatment was differentiated depending of the disease localization. The patients with local relapses in the area of the breast were administered distance radiation therapy (DRT) and 4 courses of polychemotherapy (PCT) by TC protocol (Neotaxel 175 mg/m2 + Carboplatin 250-300 mg/ m2). The patients with metastases to the brain were administered Temodal in radiomodifying dose of 75 mg/m2 per day during the course of DRT to the neurocranium, after 4 week break the patients were administered 6 courses of Temozolomide as monotherapy by the standard protocol. The patients with metastases to the skeleton were administered DRT with bisphonoites administration. The described techniques of CRT for relapses and metastases of breast cancer are moderately toxic and demonstate a favorable profile of safety, which positively influences the quality of life of these patients.

Modification of tumor individual radiosensitivity

Summary. The paper covers an urgent issue of radiobiology and radiation oncology, i.e. individual radiosensitivity. Radiobiological and clinical aspects of its formation in the tumor and normal cells and tissues are featured. The influence of various factors on formation of individual tumor radiosensitivity, which should be taken into consideration with the purpose of its modification, is described. The original experimental and clinical findings on the use of one of radiotherapy (RT) modifiers hyperthermia at multimodality RT for uterine body cancer are reported. It was proven that the use of hyperthermia, chemotherapy and RT resulted in synergism of their antiblastoma effect. The use of hyperthermia, radiomodifier in combination with RT in cancer patients allows to increase the therapeutic effect of the radiation and increase five-year survival.

HDR brachytherapy in radiation treatment for cervical cancer

Summary. Combination radiation therapy by radical program using GyneSource (Eckert and Ziegler, Bebig) high activity *°Co unit was performed with the purpose to determine optimal modes of intracavitary therapy (ICRT) for local cervical cancer (LCC). The received findings suggest high efficacy of ICKT in patients with LCC using GyneSource (HDK) units, which can be due to high accuracy of pre-radiation preparation of the patients and correct planning.

The experience of radiation therapy for stage I-III prostate cancer using linear accelerator

Summary. The authors report the experience of radiation therapy in 134 patients with stage Mil prostate cancer using linear accelerator and individual radical programs. It is shown that conformal irradiation significantly decreases radiation load to the adjacent critical organs, which results in better quality of treatment, reduction of the number and degree of radiation reactions and lesions.

The first experience of application of PET CT in oncology

Summary. The results of first experience of application in Ukraine of combined positron emission and computed tomography (PET CT) in the Center of Nuclear Medicine based at Kyiv City Clinical Cancer Hospital are presented. The findings of investigation of cancer patients with various tumors are analyzed. Importance of use of PET CT in diagnosisand staging of the disease, determining the degree of the process generalization and differential diagnosis of the character of the process is shown.

Monte-Carlo method in radiation therapy

Summary. The Monte-Carlo method is the most accurate dose calculation algorithms in radiotherapy and allows avoiding significant errors and uncertainty in creating the radiation treatment plan. Development of new, fast Monte-Carlo codes with the use of modern computers makes it possible to use the Monte-Carlo in routine practice. Own experience has shown the possibility of effective application of the method in the treatment of patients with tumors of the lung and head and neck.

The issues of radiotherapy equipment supply in Ukraine

Summary. Based on statistical data and the original experience, the authors analyze radiotherapy equipment supply in Ukraine, i.e. the number and quality of the equipment, possibility to introduce modern radiation technologies, requirement in treatment of cancer patients, and situation in the regions. The sources of irradiation using 60Co and 192Ir for brachytherapy are analyzed. The ideas about renewal of the x-ray therapy equipment stock are presented.

The prospects of treatment for highly malignant brain gliomas. Antiangiogenic therapy

Summary. Treatment for malignant brain tumors is an urgent issue of modern oncology. The prognosis of these patients is unfavorable, with median survival of 9-12 months. New treatment methods capable of increasing the relapse-free period and general survival of the patients are searched for. Modern strategy of treatment of the patients with malignant gliomas is based on interdisciplinary approach including surgery, radiation and chemotherapy, symptomatic treatment. Variants of antiangiogenic therapy have been developed recently. The results it its application are equivocal and require further investigation.

The use of chemomodification for tumor apoptosis ceramide pathway induction

Summary. Comparative analysis of the clinical findings of pre-operative radiation therapy in patients with non-small-cell lung cancer with chemomodification (Taxotere, Etoposide, Cisplatin) and without it demonstrated the advantages of the combination therapy. Experimental investigation of chemomodifying effect of chemotherapy drugs (Taxotere, Etoposide, Cisplatin) on Guerin's carcinoma showed various mechanisms of accumulation of pro-apoptosis ceramides and their potential role in apoptosis induction and tumor regression.

Post-operative IMRT in hypofractionation mode with integrated boost to the tumor bed in patients with stage I-II breast cancer

Summary. Twenty-two patients with stage I-II breast cancer were performed IMRT in hypofractionation mode with integrated boost to the tumor bed after radical surgery. Acute radiation reactions and complications were not observed during the radiation therapy.

Accompanying therapy with Licopid and melatonin at radiation therapy for uterine body cancer

Summary. The authors analyze the results of accompanying therapy with Licopid and melatonin of patients with uterine body cancer who were administered post-operative radiation therapy. The accompanying therapy limited the negative effect of radiation therapy on hematological and immune parameters and positively influenced the quality of life of the patients.

Radiation therapy of patients with typical stage IIA/В testicular seminoma

Summary. The study included 89 patients with a typical testicular seminoma of 6 months 6 years duration. Radiation therapy was performed on a liner accelerator with reduced irradiation fields. Focal dose in IIA tumor made up 30 Gy, IIB 36 Gy. A relapse-free survival constituted 97.1 and 86.4 % in IIA and IIB tumors, respectively. Cancer-specific survival comprised 100 and 95.5 %, respectively. Radiation therapy with reduced irradiation fields is considered an effective, method of treatment for stage IIA./B testicular seminoma.

Radiation therapy in complex treatmnt for local breast cancer

Summary. Breast cancer (BC) is the most common type of cancer in women. Statistical indices demonstrate a high percentage of neglected BC cases. The methods of complex treatment for local breast cancer are primarily aimed at reduction and destruction of the primary tumor with the purpose of operative treatment as well as prevention of possible subclinical metastases. Neoadjuvant endolymphatic polychemotherapy followed by small-fraction radiation therapy was considred to be more sparing method of treatmnt for normal organs and tissues.

Preliminary results of the use of photon-magnetic therapy in prevention and treatment of skin radiation reactions of patients with breast cancer with adjuvant radiation therapy

Summary. The authors report preliminary findings of the investigation of the effect of combination photon-magnetic therapy with successive application of red and blue light to the skin of breast cancer patients during the course of post-operative radiation therapy. It was established that photonmagnetic therapy positively influenced the state of the skin in the irradiated areas. Addition of the magnetic factor significantly improved the efficacy of phototherapy. The patients receiving photon-magnetic therapy finished the course of radiation therapy with almost unchanged skin.

Immediate results of treatment for primarily inoperable breast cancer using fluoropyrimidines

Summary. The findings of the research of the efficacy of neoadjuvant complex antitumor therapy using chemoradiomodifier of fluoropyrimidine group are presented. Immediate results of treatment for local breast cancer using the ordinal and standard methods are analyzed. It was established that neoadjuvant chemoradiation therapy (CRT) against a background of the modifier allowed to increase by 30% the number of the primary tumor responses to treatment, namely, the cases of partial and complete regression as well as increased the frequency of surgical interventions. Addition of fluoropyrimidines to the treatment protocol did not increase the treatment toxicity. Investigation of relapse-free survival revealed a tendency to its probable increase in patients from the study group who were administered CRT against a background of fluoropyrimidines.

Assessment of efficacy of chemotherapy for breast cancer using 99mТс-MIBI scintimammography

Summary. Nineteen women with breast cancer underwent 99mТс-MIBI dynamic scintiinaininography with the purpose to define most optima] parameters the RP uptake and excretion. The most informative indicators of RP kinetics were identified. A clear correlation was found between the RP uptake and tumor size. No correlation could be found between the time to half clearance and tumor size or differentiation degree.

Radiosurgical treatment of metastatic liver lesion with the use of robotic CyberKnife G4 system

Summary. The capabilities of stereotactic radiosurgery allow highly precise delivering of lethal tumor dose to the pathological focus without extending radiation exposure tolerance parameters onto the surrounding healthy tissues. Twenty-four patients with metastases to the liver underwent treatment. Metastases of colorectal cancer to the liver appeared to be most sensitive to the radiosurgical treatment.

Radiosurgery for brain meningiomas using CyberKnife G4 system. Results and their radiology assessment

Summary. Stereotaxic radiosurgery with the use of CyberKnife G4 system is an effective method of treatment for brain meningiomas. Objective assessment of the treatment results is possible with standard radiology techniques (CT, MRI) supplemented by special MRI modes (DWI) and determining ADC coefficient.

Efficacy of radiosensitizer Hydrea at radiochemotherapy for local cervical cancer

Summary. Long-term results of radiochemotherapy with Hydrea as a radiosensitizer for local cervical cancer were obtained. The authors conclude that this treatment promotes improvement of the immediate results by 25.7%, increases the incidence of complete regressions of stage 3NxM0 tumors and survival level in patients without the signs of the tumor process within a five-year period by 8-10% due to 1.69 times reduction of the incidence of locoregional relapses.

Complex monitoring of cervical tumor resorption during combination radiation therapy

Summary. Complex ultrasound examination with Doppler investigation was performed in 48 patients with stage II—III cervical cancer during radiation therapy including brachytherapy with Co-60 source and high dose rate with MultiSourse unit. Complex monitoring allows to assess objectively the tumor response and predetermines optimizing dose-time ratio of the components of combination radiation therapy.

Complex application of CT and ultrasonography in diagnosis of secondary liver lesions

Summary. Efficacy of complex application of CT and ultrasonography in diagnosis of secondary liver lesions was assessed. It was established that only complex application of computed tomography with contrast enhancement and ultrasonography can give complete information for diagnosis of secondary liver lesions. Diagnostic value of CT without contrast enhancement is very low while radiation load to the patients is unreasonably high.

Prognostic significance of intercellular adhesion markers expression in patients with cancer of oropharynx

The level of expression of intercellular adhesion markers (E-cadherin and β-catenin) in patients with squamous cell cancer of the oropharynx was performed with the purpose to reveal clinical morphological associations and analyze their significance for the efficacy of chemoradiation therapy. Prognostic significance of each marker as to the effect of radiation therapy was established.

The analysis of combination treatment for gastric cancer depending on the tumor location

Summary. The results of combination treatment for gastric cancer are reported depending on the tumor location. Radiation therapy in the mode of medium fractionation was well tolerated. Increased doses up to 54-56 Gy allowed to improve long-term results and did not cause delay in surgery. The most prognostically favorable location is lower portion of the stomach.

Clinical peculiarities of the course of early and late postchemioradiation pulmonary lesions in patients with breast cancer

Summary. The paper features the investigation of pulmonary complications after chemoradiation therapy in patients with breast cancer. It was found that respiratory system lesions developed in 76.7 % of patients. Early lesions, mainly bronchitis and pulmonitis, developed 3 months after the beginning of treatment in 63.4 % of patients, were accompanied by dyspnea, cough, inflammatory phenomena, reduction of oxygen saturation and external respiratory parameters. After 6 months inflammatory phenomena subsided, the function of breathing restored. After 12 months SCT demonstrated pneumosclerosis and fibrosis in 33 % of patients, they were accompanied by a significant reduction of external respiration (forced capacity, maximum lung ventilation, peak expiratory volumetric rate and oxygen saturation) and secondary inflammation. Clinical manifestations of late postchemoradiation complications do not always correspond to x-ray images; therefore SCT plays an important role in early diagnosis of pneumosclerosis and pneumofibrosis.
