THE IMPORTANCE OF DETERMINING THE LEVELS OF β2-MICROGLOBULIN AND THYMIDINE KINASE FOR TREATMENT STRATIFICATION IN PATIENTS WITH ACUTE MYELOID LEUKEMIA AND ACUTE LYMPHOBLASTIC LEUKEMIA

The article presents the results of determining the levels of TC and β2-IG in blood serum of patients with AML and ALL during their treatment. It is proved that the definition of the initial levels of Tc and β2-IG in blood serum enables evaluating the malignancy of the tumor clone in order to plan the further chemotherapy. Initial values of TC and β2-IG can be used prognostically in patients with AML and ALL to divide them into risk groups for further treatment stratification. Determination of TC dynamics is useful for evaluating the effectiveness of chemotherapy: the normalization of its level confirms a remission, whereas the increase is an evidence of the relapse. Patients with initial TC levels above 20 U / l, and P2-IG more than 10 mg / l should be subjected to more intensive chemotherapy programs.

RADIOLOGICAL PECULIARITIES CHANGES IN LUNGS DURING PNEUMOCYSTOSIS IN CHILDREN WITH AIDS

Objectives. To investigate the radiological peculiarities of lungs changes in pneumocystosis in children with AIDS.

Materials and Methods. The data, obtained by means of X-ray images in frontal and additional projections of the chest in 42 children with AIDS aged from 6 to 18 at Kharkiv Regional Children’s Infectious Diseases Hospital, were studied. The effective dose in this case was 2.2-8.5 mcSV/mAc. The diagnosis was verified by clinical laboratory testing.

Results. The data analysis made it possible into 3 radiological.

The first which appears at the beginning of pneumocystic pneumonia is not specific; changes of lung pattern are observed on X-ray images. First 5-8 days were characterized by intensifying of juxtahilar regions (76.2 %) and further it spread to both lungs.

The second was characterized by progression in the form of inflammation during 5-10 days. During next 3-4 weeks diffuse lateral, juxtahilar interstitial infiltrates, inferobasal predodominantly, spreading from the roots to periphery («frosted glass», «blizzard», «cotton lung» symptom) were radiologically apparent. These changes were detected in 30 % of patients and corresponded to atelectatic phase. Pneumonia in children with AIDS was distinct in diffusion nature and frequent involvement of intrathoracic lymph nodes.

The third reflected pleura changes. X-ray images of 33.3 % showed induration of pleura along the horizontal fissure on the right. In 9.5 % a course of bilateral pneumonia was complicated by pleurisy with low amount of liquid. Conclusions. Chest X-ray is a basic method of diagnosis of changes in the lungs in children with AIDS. The peculiarities of radiological manifestations of a current course of pneumocystic pneumonia from initial changes of the lung pattern to more intensified presentation have been established as well as complications and dynamic in treatment have been reported.

ULTRASOUND SCREENING OF CHROMOSOMAL PATHOLOGY IN PREGNANT WOMEN IN ASSISTED REPRODUCTION IN II TRIMESTER

The aim of our retrospective study was to evaluate the differences, and hence the effectiveness of ultrasound screening markers in the II trimester in women with complicated reproductive history (after ART) and in physiological pregnancy. For this, we analyzed the results of ultrasound screening in II trimester in 1322 women after ART and 936 in spontaneous pregnancies. Revealed that quantitative (nuchal thickness, length of nasal bone, proportion of prenasal skin thickness to the length nasal bone; nasal bone/biparietal diameter) and qualitative (frequency of ventriculomegaly, hyperechogenic focus in the left ventricle of the heart, pielectasis, single umbilical artery) ultrasound markers of chromosomal pathology in II trimester are informative for both spontaneous and induced pregnancies. Resistance of blood flow in the ductus venosus and uterine arteries in women with induced pregnancies may be associated with etiopathogenesis of infertility.

DOPPLER AND X-RAY PATTERNS IN PATIENTS WITH CERVICOGENIC HEADACHE

Purpose. To study the state of cervical spine and arterial and venous hemodynamics in vessels of vertebrobasillar system in young patients with various kinds of headache.

Materials and Methods. We have studied 75 patients (41 female and 34 male) of young age (18-35) with headache on the background of cervical spine pathology. Cervicogenic headache (CH) manifested itself as cervicocranialgia (CCA) and Barre-Lieou posterior cervical sympatic syndrome (BLS). Using the method of transcranial Doppler (TCD), we have studied the values of blood flow linear velocity (BFLV) in vertebral (VA) and basilar (BA) arteries, vertebral veins (VV), Rosenthal basal veins (RBV) and tentorial sinus (TS) at rest and under functional loads. All patients received functional X-ray study of cervical spine with bending and unbending.

Results and discussion. X-ray study of patients with CH more often revealed the signs of initial osteochondrosis of cervical spine and instability in one or several motion segments. All patients with scalene instability manifested hyperreactivity to tests with bending and unbending and rotation loads combined with regional changes of hemodynamics in BA and one or both VAs. Patients with CCA manifested a vasospasm in both VAs in 26.2 % of cases, vasospasm in one SA and/or BA in 20.8 %, blood flow asymmetry (25-30 %) through VA in 17.6 %. With BLS vasospasm in one VA was recorded in 44.7 % of cases, combination of vasospasm in BA and one VA in 42.4 %. In 68.9 % patients, we observed the increase of systolic BFLV in VA, in 47.2 % — in TS and in 18.6 % of patients — in BV. With BLS, we more often observed hyper perfusion (31.5 % of cases). At the orthostatic test, we recorded hyperreactivity in VV in both clinical groups compared to the control set, more pronounced in patients with CCA.

Conclusions. Cervicogenic headache in young patients is mainly determined by the scalene instability of cervical spine. In patients with CCA venous discirculation in VV and TS dominates, in patients with BLS — vasospastic reactions in VA and BA, and, to lesser extent — discirculation in VV Hyperreactivity to rotation tests correlates with the presence of cervical spine instability. It is necessary to perform a complex Doppler and X-ray study of all patients with supposed cervicogenic headache.

THE CASE OF TREATMENT OF RECURRENT CANCER OF SKIN OF HAIRY PART OF THE HEAD COMPLICATED WITH RADIATION INJURIES

The paper presents a case of surgical treatment of recurrent cancer of skin of hairy part of the head in combination with late radiation skin necrosis, osteomyelitis of the parietooccipital cyst and radiation encephalopathy of the parasaggital region of the brain with lower paraparesis, that occurred after radiation therapy of fungi-shaped form of locally advanced cancer of skin of calvaria (a short-focused X-ray therapy with a total dose of 60 Gy). High ability of malignant tumors of calvaria soft tissues to recur was shown, as well as complications after a short-focused X-ray therapy. The clinical observations showed the possibility of surgical treatment and a higher radicality of combination of surgical and radiological methods in locally advanced cancer. These results are in agreement with literature data concerning the leading role of surgical treatment of malignant tumors of calvaria soft tissues.

VASCULAR TARGETS OF CANCER TUMORS FOR RADIOTHERAPY

One of the most important promoters of angiogenesis is considered to be the vascular endothelial growth factor (VEGF) which stimulates formation, growth and permeability of blood vessels. Radiation-induced VEGF activation subsequently attenuates vasculature damage of blood vessels and leads to reduced tumor cytotoxicity. It has been shown that combination of antiangiogenic agents with radiation treatment has the potential to enhance tumor cytotoxicity. This article also focuses on potential targets for radiation therapy, which are VEGF inhibitors, as well as eicosanoid, sphingolipid and ceramide pathways.

In a review, the history of openings and studies of radioprotectors and the modern going of classification of the radioprotection drugs are resulted. There are examined the mechanisms of realization of radioprotective action of «classic» radioprotectors, radioprotectors II type and radiomodificators. It is grounded, that modern conceptions of radioprotection are based on the on principle different «points of application» of groups of radioprotectors and depend on the stage of radiation injury. There are discussed the prospects of application of radioprotectors in clinical practice.

PERSPECTIVE OF SPECTROMETRY IN DETERMINATION OF THE COMPATIBILITY AND EFFECTIVENESS OF ANALGESIA COMPONENTS IN PATIENTS WITH RADIOLOGICALLY PROFILE

Spectrophotometry method was used to determine the stability of bupivacaine hydrochloride in the original condition and with different lipid environments on the basis of available medicines (Lipin, Lipofundin, Gelofusine). The optical spectrum of the lipid fractions of bupivacaine was determined in dynamics. The identity of the character of the optical spectrum absorption of the samples in the UV region was established. The greatest stability for bupivacaine hydrochloride is indicated in the region of maximum absorption at X=220, X=262 and X=271 nm for the lipid environment of Lipofundin. The potential of the solution for prolonged anesthesia for a category of cancer patients is demonstrated.
