Comparison of 4 and 128-slice MHCT at CT angiography of brain vessels in patients with arterial hypertension

Objective: To compare the diagnostic information about the state of the brain vessels in patients with arterial hypertension obtained at investigations using 4 and 128-slice CT.
Material and Methods: The study involved 88 patients aged 21-75 who were divided into 4 groups depending on the level of the total cardiovascular risk: 62 patients were investigated using 4-sliceCT and 26 128-slice CT. At 4-slice MHCT, protocol Gerebral CTA was used in 38 patients, Sure Start program in 24 patients. At 128-slice CT Neuro DSACT program was used in 11 patients, Carotid DSACT in 15 patients.
Results: The following changes of the intracranial vessels were observed in patients with arterial hypertension: narrowing of the vertebral artery (44 patients, 50 % ), S-like deviation of the inner carotid artery (ICA) (22 patients, 25 %), curved ICA (25 patients, 28.41%), narrowed ICA (12 patients, (13.4 %), narrowed anterior cerebral artery (20 patients, 32.95 %). Twenty-three cases (26.14 %) of brain vessel aplasia and 37 cases (42.04 %) of brain vessel hypoplasia were registered.
Conclusion: MHCT angiography in patients with arterial hypertension allows accurate and reliable detection of the changes in the brain vessels. The advantages of 128-slice CT is the use of smaller amounts of contrast substance, on an average by 30%, reduction of irradiation dose by 47.4%, as well as larger area of scanning when compared with 4-slice CT.

Liver-specific lipoprotein as a possible marker of immune tolerance arrest under the influence of low-dose ionizing radiation

Objective: To investigate the role of liver-specific lipoprotein (LSL) in formation of autoimmune liver lesions under the influence of low-dose ionizing radiation (IR).
Material and Methods: The following methods were used in the study: immunoenzyme essay, affine chromatography, immune blotting, and electrophoresis in polyacrylate gel in the conditions of denaturation.
Results: It was shown that prolonged influence of low-dose low-intensity ionizing radiation was associated with increase of serum levels of autoantibodies to LSL. These levels were close to those of autoantibodies (AAB) to LSL found in patients with chronic hepatitis.
Conclusion: Prolonged influence of low-dose low-intensity IR associated with increase of serum AAB to LSL can suggest about a negative effect of low-dose IR on the immune tolerance of the organism and explain the increase in the exposed population of the disease frequency of the diseases with an important role of autoimmunity reactions in the disease pathogenesis.

Application of Noni juice as antiradiation remedy for prevention and treatment of radiation lesions of the mucous membranes of the mouth and eyes at total x-ray exposure

Objective: To assess the influence of Noni juice administered orally on the processes of inflammation of the oral mucosa and eyes in rats exposed to total x-ray irradiation at a dose of 6.0 Gy.
Material and Methods: The study involved 146 mongrel female rats weighing 160-190 g. Total x-ray exposure at a doe of 6.0 Gy was delivered by РУМ-17М unit. Accumulated fre quency of the signs of radiation lesion of the mucous mem braines of the mouth and eyes within 30 days of the experi ments with oral administration of Noni juice (Tabari, Hawai ian) was determined.
Results: Administration of Noni juice to irradiated animals reduced 30-day mortality, frequency of radiation lesions of the mucous membranes of the head in the survived animals and increased it slightly in the group of dying animals.
Conclusion: The investigated samples of Noni juice ad ministered orally reduced not only the manifestations of acute radiation sickness in the rats but also local inflammatory processes in the mucous membranes of the head. This is clearly seen in the survived animals. When assessing the mechanism of action of antiradiation remedies on the irradiated rats, it is necessary to consider their influence on both surviving and dying animals.

Summary. Thyroglobulin (TG) level is an important prog nostic factor during treatment and monitoring of the patients with differentiated thyroid cancer (DTC). Of 150 investigated patients, 32 with increased TG level were selected. Complex radiation diagnosis was doe, dynamics of TH level was deter mined. Association of TG level and peculiarities of DTC course was analyzed. The indications for further antitumor treatment were determined.

Five-year relapse-free survival after combination treatment for differentiated thyroid cancer

Summary. The results of treatment of patients with diffuse thyroid gland cancer with radioactive iodine as one of the constituents of the combination treatment are shown. Ana lysis of 5-year clinical follow up of 81 patients with diffuse thyroid cancer, treated at Odessa Regional Cancer Hospital, was done. Radioiodine therapy of patients with diffuse thyroid gland cancer is an integral part of combination treatment allowing considerable increase of the duration of life and improvement of the quality of life of the patients.

Thyroid peroxidase in the punctuates of radioiodine-responsive and radioiodine-resistant metastases of the papillary thyroid cancer.

Summary. The immunocytochemical detection of the thyroid peroxidase in the punctuates of two groups metasta ses (radioiodine-responsive and radioiodine-resistant) was performed. The reliable difference of the thyroperoxidase positive thyrocytes percentage between two groups was establis hed. We could recommend using the immunocytochamical detection of the thyroperoxidase-positive thyrocytes in the punctates of thyroid papillary metastases found in post operation period. This approach allows predict response of metastases on radioiodine therapy to choose proper therapeutic approach.

Radiology verification of pre-clinical forms of hemodynamic disorders in men with metabolic syndrome and type II diabetes mellitus

Summary. Investigation of the state of blood flow in lower extremities, kidneys, brain tissue, and testes of 85 men with metabolic syndrome and type 2 diabetes inellitus aged 30-65 performed with scintillation homographic gamma-camera ГКС-301Т (Orizon) showed that microangiopathy preceded inacroangiopathy development and was diagnosed by radio nuclide methods before appearance of clinical signs.

The use of indirect radionuclide cystography in radiology departments

Summary. Based on the original findings the authors show that application of IRCG is reasonable in detection of CUR at investigation of the children with the history of urinary system infections, chronic pyelonephritis, neurogenic bladder, development defects, etc. This method should be included in the complex of examination of the children both with su spected CUR and at dynamic observation of the patients with CUR.

Comparative characteristics of bone scintigraphy with Tc-99m pyrophosphatе and x-ray diagnosis in patients with revison of femur joint grafting

Summary. Seventy eight patients with revison femur joint grafting aged 31-75 were investigated with the purpose to compare bone scintigraphy with Tc-99in pyrophosphate and x-ray diagnosis. Bone scintigraphy in comparison with x-ray diagnosis is a method of the priority for determination of the inflam matory process in the femur joint. Bone scan can be used to determine the stage inflammatory process in the femur joint, as well as for monitoring the result of their surgical treat ment.

Informativity of nephrotropic radiopharmaceuticals in assessment of the degree of kidney involvement at various nephropathies in children

Summary. The performed investigations showed that in acute and chronic pyelonephritis the most informative NTRP are 99mTc-DTPA and 99mTc-DMSA. To assess the function of a single kidney, at hypoplasia and dysplasia of the kidneys, and obstruction, all NTRP are informative. 99mTc-EC is more pre ferable at cystourethral reflux. The choice of NTRP for assessment of hemodynamic disor ders, the function and structure of the kidneys should be determined by the tasks of the investigation, mechanism of the radiopharmaceutical elimination, nosological form of the disease, its character and severity.

Modern aspects of application of three-phase bone scan in diagnosis of locomotor system disorders

Summary. The technique of З-phase bone scan (3pBS) is featured; the indications to its application are described. The peculiarities of scintigraphy data and their interpreting at some benign pathological processes of the locomotor system (osteomyelitis, traumatic lesions of the bones, vascular necro sis, etc.) are analyzed. The findings are compared with those of other radiation methods of investigation, in which 3pBS is most informative. The advantages of the method were determined.

Hyperplastic processes in endometrium and breast in women after radioiodine therapy

Summary. The results of ultrasound monitoring of the uterine, ovarian, and breast structures in 50 patients in dynamics of long-term follow-up after thyroidectomy and radioiodine therapy for differentiated thyroid cancer are presented. It was shown that starting with postoperative period the signs of lutein phase insufficiency are determined in them. It was established that acute changes of thyroid status as a result of thyroidectomy and radioiodine therapy were accom panied by development of hyperestrogenemia in the examined women, which results in the development of dyshormonal breast hyperplasia, hyperplastic processes in endometrium, and increased frequency of genital endometriosis. Correction of providing menstrual cycle in women after thyroidectomy and radioiodine therapy is required.

Comparative investigation of tumor markers of thymidinkinase β2-microglobulin as prognostic factors in patients with acute myoblast leukemia

Summary. The author presents the findings of determining the levels of TK and β2 MG in the blood serum of the patients with AML during the treatment using a standard protocol of remission induction 7+3. It was proven that TK was an inde pendent prognostic factor of AML malignant course and an important criterion of the treatment efficacy. Its parameters did not correlate with the amount of leukocytes in the blood and blast cells in the bone marrow, β2 MG is not an independent prognostic factor of the course of AML and response to CT but is an additional criterion of the treatment efficacy. Its level does not possess differential diagnosis significance in iden tification of AML variants but can be used as a criterion of remission completeness and in early diagnosis of relapses. It was determined that the lower are the initial indices of TK and β2 MG, the higher is remission probability. In cases when it is not complete, TK and β2 MG levels do not achieve the normal parameters, even if they decrease.

The peculiarities of regional volume cerebral blood flow in patients with hypertensive encephalopathy by the findings of single photon emission computed tomography

Summary. The investigation of the state of the brain (B) perfusion in patients with stage II hypertensive encepha lopathy (HE) at development of hypertension crisis (HC) vs. crisis-free course of HE showed that in 79.5% of the patients with stage II HE, SPECT demonstrated local changes of the blood flow. In those who survived crisis, disorders of brain blood flow are more pronounced than in patients without HE complications. This was proven by low hemisphere OMK, presence of larger amounts of local perfusion reduction with lower KA in the foci and more frequent visualization of crosscerebral diaschisis.

Some peculiarities in the content of estradiol in adolescent girls with secondary amenorrhea

Summary. The purpose of the work was to study the content of blood estradiol in adolescent girls with secondary amenorrhea (SA). Eighty girls aged 13-17 with SA and 78 girls with regular menstruations were examined the rates of blood estradiol were investigated. In adolescent girls with SA estradiol level may be normal and increased. SA may be the result of receptiveness metrosis as regards the hormone, or the consequence of strong relations between estradiol and sex hormone-binding globulin.

The technique of preparation of the patients to RIT at thyro toxicosis is described. Immediate and long-term results of RIT are analyzed. Radionuclide diagnosis of diabetic nephropathy

Summary. The authors report the results of investigation of the state of filtration-excretory function of the kidneys, renal hemodynamics in patients with type 1 diabetes mellitus and stage II and III diabetic nephropathy. The changes in filtration-excretory function and renal hemodynamics manifested by various changes of minute volume of glomerular filtration, maximal accumulation time and half-excretion of the radiophamaceutical as well as reduction of arterial in-flow and venous out-flow in patients with pre-clinical stages of diabetic nephropathy. The changes in the parameters of dynamic kidney scan and intrarenal hemodynamics were observed in patients with diabetes mellitus with stage II diabetic nephropathy and absence of albuminuria.

Dynamics of circulating immune complexes in patients of different age with differentiated thyroid cancer in the process of radioiodine therapy

Summary. Determining the influence of radioiodine on the amount of circulating immune complexes in the blood serum of patients of different ages demonstrated patients' age did not have any significant effect on the degree of changes in CIC content in patients who underwent radioiodine therapy, though in younger patients immunological disturbances were reported in earlier terms.

Early sings of heart failure in patients with of cardiovascular diseases (radionuclide ventriculography findings)

Summary. Comparative analysis of main parameters of systolic and diastolic function of left ventricle (LV) in patents with coronary artery disease (CAD), dilated (DCMP) and hyper trophic (HCMP) cardiomyopathy was done. Heart function was examined by cardiosynchronized gated radionuclude ven triculography. Heath function in CAD patients was charac terized by preserved pumping funcrion, contractility reduc tion, and disturbance of LV diastolic funtion. Heart failure in CAD patients may be described as combined (systolic diastolic) form. Heart function in HCMP patient was characterized by preserved systolic function and disturbance of diastolic function. Heart failure in HCMP patients may be described as diastolic form of heart failure. Heart function in DCMP patients was characterrized by considerable reduction of systolic and relativery less distur bance of LV diastolic function. Heart failure in DCMP may be described as preferential systolic form.

The analysis of findings of dynamic hepatobiliscintigraphy in various diseases of the biliary system

Summary. The capabilities of dynamic hepatobiliscinti graphy are shown in revealing and dynamic observation of the patients with the liver and biliary system pathology. It is noted that at the process progression in patients with hepato biliary pathology, both the liver parenchyma and biliary tracts are involved in the process irrespective of the hyper or hypo kinetic character of the state. The degree of the hepatobiliary system involvement directly correlates with the degree of the main process activity, its duration, age of the patients and does not depend on the administered treatment.

Complex scintigraphy of children with cystourethral reflux

Summary. The purpose of the present study was to work out and suggest a technique of complex scintigraphic examination (CSE) using radiopharmaceuticals (RP) with various mecha nisms of elimination and fixation in the kidneys and to im prove the efficacy of early diagnosis of cystourethral reflux (CUR) in children. The study suggests that the most sensitive method of CUR diagnosis is IRCG, combination of standard DRCG and IRCG yields more significant and accurate findings in diagnosis of CUR, especially for dynamic observation, which allows considerable reduction of radiation load on the children.

Analysis of morbidity and treatment quality of cardiovascular and thyroid diseases in the professionals working with ionizing radiation (follow-up data)

Summary. Cardiovascular (CV) and thyroid (T) morbidity as well as their combinations were analyzed in the group of patients working with ionizing radiation (IR) using the data of Central Dispensary Commission at S.P. Grigoriev Institute for Medical Radiology (National Academy of Medical Sciences of Ukraine). It was revealed that the frequency of CV diseases in this group did not differ from that in the general population, in contrast to T diseases, the level of which 1.5 times exceeded the indices in the general population. The analysis of administered treatment suggested that it corresponded to the national standards but it is necessary to strengthen control of the follow-up of the professionals at out patient stage. Further investigation of the state of health of the persons professionally associated with IR exposure is necessary for understanding the problem of the effect of low dose radiation on the human organism, optimization of dose load on the workers, which could promote improvement of the prognosis of health in this category of patients.

Complex radiation diagnosis in breast cancer screening

Summary. It is shown that complex use of mammography and ultrasonography at screening for BC allows increase in the number of detected cases especially in women with non-palpable tumors. To improve early diagnosis of BC it is necessary to improve the patients follow-up by means of organization and opening of mammography centers based in not only in cancer hospitals but also in general hospitals.

Personnel radiation safety. A case of hand lesion in a radiologist

Summary. The work featured the questions of norms and rules of radiation safety at work with ionizing radiation. The history of the question about the permissible doses is described. The changes in the skin when exceeding the tolerant dose are described. A case of severe local lesions of the hand caused by chronic occupational overirradiation, when the safety rules were neglected, is described.

The experience of periodical medical check-ups of persons working with ionizing radiation sources in Zaporizhzhia region

Summary. The work features the organization and proce dure of periodical medical check-ups of the persons working with ionizing radiation sources. Somatic pathology was analyzed in various professional groups. Increase of cardio vascular and gynecological pathology within the recent 5 years was detected. The problems in the work of follow-up com missions are described.
