RAYS: 120 YEARS OF TRIUMPH AND DISCUSSIONS

Summary. The main factors facilitating the scientific and public recognition of W.K. Roentgen’s contribution to development of physical and medical sciences have been analyzed along with features of precedence recognition in discovery of X-rays. The main types of of cathode tubes, used by scientists dealing with cathode rays in the latter half of the twentieth century, were considered and distinguished. The enduring role of powerful scientific environment concerned with promotion and further efficient emoloying of W.K. Roentgen’s research oiutcomes in fundamental physical studies as well as in numerous applied aspects was shown in terms of XR discovery.

POSTOPERATIVE CHEMORADIOTHERAPY WITH DIFFERENT TOTAL DOSES OF RADIOTHERAPY AND TEMOZOLOMIDE FOR GLIOBLASTOMA PATIENTS

Objective. Evaluating the long-term outcomes of employing different total target doses (TTD) of radiation treatment and temozolomide in multimodality therapy of glioblastoma patients.
Materials and methods. The study involved 258 patients aged from 16 to 77 with Karnofsky performance index ≥ 50%. In their treatment, the TTD range was 54–60 Gy, the range of temozolomide total dose was 2000–7400 mg. Outcomes. The TTD increase from 54–57 Gy to 58–60 Gy in patients aged ≤ 60 years raised the median survival from 15 to 19 months, and 5-year survival from 5.9% to 13.0% (P=0.003). No improvement in survival due to the TTD increase was noted in patients older than 60. The relationship between the treatment outcomes and different temozolomide total doses was not revealed.
Conclusions. The TTD increase from 54–57 Gy to 58–60 Gy in postoperative chemoradiotherapy for glioblastoma patients aged ≤ 60 years improves the treatment Outcomes. The relationship between the treatment outcomes and different temozolomide total doses was not revealed.

CLINICAL AND LABORATORY FINDINGS IN PATHOGENESIS OF FORMATION OF DENTAL PATHOLOGY IN CHORNOBYL PATIENT POPULATION

The study deals with dental status and mineral metabolism indices in people exposed to Chornobyl NPP accident in high and low radiation doses. Dental status, biochemical indices of blood, urine, saliva as well as tooth histology were studied. Tooth decay, high concentrations of Ca, chondroitin sulfate, parathormone and low level of calcitonin in blood, high indices of oxyproline and hexuronic acid in urine resulting from high radiation doses were observed, where severity depended on the dose received. Low radiation doses cause multiple caries and severe forms of parodontitis. Increased level of Ca and low concentarion of inorganic phosphorus, alkaline and acid phosphatase as well as α-amylase were revealed in saliva. Calcium-phosphorus metabolism in blood was impaired. Development of dystrophic changes in the pulp along with blocking of remineralization in dental tissues and bone tissue were observed.

THE CLINICAL SIGNIFICANCE OF VEGF IN THE SERUM OF PATIENTS WITH SECONDARY EDEMATOUS BREAST CANCER

The purpose of the study is to ascertain possible interaction between the content of blood VEGF angiogenesis marker in patients with secondary edematous breast cancer (SEBC) and the main clinical morphological characteristics of the disease.
Materials and methods. The study involved 20 women with SEBC aged from 38 to 75, undergoing treatment at
S. P. Grigoriev Institute for Medical Radiology from January 2015 to July 2016.The control group was made up by 10 healthy women with due regard for age criteria. The content of serum VEGF in blood serum was assessed by means of enzyme immunoassay using «Vector-best» standard kits (Russia) before treatment.
Outcomes.The content of VEGF in blood serum in examined SEBC patients before the onset of specific treatment was 3.4 times higher than that one in the control group (p < 0.05). The expression of VEGF in SEBC patients was associated with prognostic factors (tumor malignancy and proliferative tumor activity (Ki-67), hormoneresistance (lack of steroid hormone receptors) and overexpression of Her-2/neu).
Conclusions. It has been proved that the biological activity of the tumor is in direct dependence on increased content of VEGF in blood serum of patients. It can be used as a factor of unfavorable prognosis of the disease.

POST-SURGICAL CHANGES IN HEART RATE VARIABILITY IN THE ENDOMETRIAL CANCER PATIENTS

Purpose of the study. To reveal peculiarities of autonomic regulation disorders after surgical treatment of endometrial cancer patients with different types of autonomic regulation identified by heart rate variability (HRV) indices.
Materials and methods. The study enrolled 21 patients with stages I–II endometrial cancer (EC) (T1а-bN0M0 – T2a-bN0M0) who had undergone total hysterectomy. Heart rate variability was studied due to Polyspectrum diagnostic set. Heart rate variability was analyzed by means of methods of temporal and spectral analysis and the variation pulsometry method provided for the patients before treatment and in 7-8 days postoperatively. Heart rate variability indices were analyzed in the groups of patients which were initially different by stress index values.
Outcomes. Pre-treatment heart rate variability indices differed significantly in the patients with different types of autonomic regulation. The lowermost values of HRV temporal and spectral analysis were recorded in the patients with high stress index value of regulatory systems. Within the period of 7-8 days after surgery, the decrease of these indices in the patients with prevailing parasympathetic tone and with balanced impact of sympathetic and parasym pathetic autonomic nervous systems was revealed. Statistically significant decrease in those patients with prevailing sympathetic tone was established only for HFn and RMSSD values.
Conclusion. Surgical treatment considerably influences HRV values. Significant decrease of HRV time and spectral domains parameters are recorded after surgical intervention in the patients with prevailing parasympathetic tone and with balanced impact of sympathetic and parasympathetic divisions of the autonomic nervous system. Essential HRV decrease after surgical treatment of endometrial cancer patients calls for perioperative rehabilitation measures, aimed at shortening the duration and expressiveness of the body’s inflammatory response.

INFLUENCE OF CONFORMAL RADIOTHERAPY ON DEVELOPMENT OF RADIATION PULMONITIS IN PATIENTS WITH NON-SMALL CELL LUNG CANCER

The purpose of the study was to assess the reaction of healthy lung tissue during conformal radiation therapy of NSCLC depending on clinical characteristics.
Materials and methods. The study involved 58 patients who had received radiotherapy by means of the linear accelerator Clinac 600 C according to the scheme of sequential chemoradiotherapy with classic fractionation of TBD 60–66 Gy. Patients were divided into 2 subgroups: Subgroup 1 was made up of 30 patients with planning of conformally irradiated target according to CT results before special treatment (average РТV – 298 mm3); Subgroup 2 included 28 patients with planning according to CT results after prior chemotherapy (average РТV – 210 mm3). 94.1 % of patients were males with the average age of 62.5. All groups were characterized by dominated stage IIIA of NSCLC, epidermoid carcinoma, central clinical and anatomical form. Non–specific lung diseases were observed in 25.8 % of cases. Most patients’ general condition was estimated as 70 points in accordance with the Karnofsky scale.
Outcomes. Radiation pulmonitis of stage I–II was diagnosed 2 times less frequently in Subgroup 2 (17.9 vs 33.3 %), which to some extent depended on the planned amount of radiation. Due to the study it was found that radiation pulmonitis was less frequently diagnosed in NSCLC stage IIIA (11.1 vs 45.5 %) along with the general condition of 70 or more points (13.7 vs. 57.1 %) according to the Karnofsky scale, in the absence of non–specific lung diseases (9.3 vs 66.8%) (p < 0.05). The development of radiation pulmonitis was independent of patient’s age, clinical and anatomical shape of the tumor and its morphological type. The increased frequency of radiation pulmonitis of stage IIIB, general condition of KS – 70 points and pulmonary comorbidity are associated with profound physiological disorders both in pulmonary system and throughout the body.
Conclusions: Conformal radiation therapy by means of the linear accelerator with the planning according to CT after CTx makes it possible to reduce the frequency and severity of radiation pulmonitis. The incidence of radiation pulmonitis increases by 4 times with NSCLC stage IIIB; in the general condition of the patient at less than 70 points – by 4 times, with concomitant chronic diseases of the pulmonary system – by 7 times, requiring an adequate supporting therapy.

INFORMATION MODEL OF THE DATA BASE FOR CATAMNESIS OF THE PATIENTS WITH THYROID CANCER AND HYPERTHYROIDISM TO USE IN A RETROSPECTIVE STUDY CASE-CONTROL

Purpose. Substantiation of the information model of database of patients with thyroid cancer aimed at analytical study of possible consequences of pathology treatment.
Materials and methods. Catamnesis information based on paper clinical records concerned with the course and consequences of thyroid cancer treatment.
Outcomes. In order to carry out the analytical study of possible effects derived from treatment of thyroid cancer, an information model of follow-up database made up of paper clinical records has been developed.
Conclusions. The study of paper clinical records was based on identification of such issues as first tumor characteristics, description of comorbidities, heredity, treatment algorithm (surgery, radionuclide therapy, radiation therapy, chemotherapy, hormone therapy), long-term oncological effects (secondary tumor, metastases, relapses), remote therapeutic effects  (dermatological survey findings, therapeutic examination findings as well  as those ones obtained by neurologist, ophthalmologist, ENT doctor, gynecologist, endocrinologist; examination of the respiratory and urogenital system, detection of GI tract diseases, nervous system diseases, musculoskeletal apparatus and visual organ disorders).

ASSESSMENT OF PNEUMONIA SEVERITY IN CHILDREN WITH CONNECTIVE TISSUE DYSPLASIA BY MEANS OF  X-RAY DATA

Objective. Improving the diagnostic criteria of pneumonia severity in children with connective tissue dysplasia using X-ray method.
Materials and methods. The chest X-ray (CXR) data of 30 children aged from 5 to 17 with connective tissue dysplasia diagnosed with community-acquired pneumonia, were studied. CXR was carried by РУМ-20М unit. The diagnosis was verified in all patients using full clinical and laboratory examination.
Outcomes. The suggested method of XR assessing of severity of pneumonia in children with connective tissue dysplasia, carried out in conjunction with metric method estimation of prevalence of infiltration, makes it possible to reveal inflammatory process over time. Due to the analysis of CXR data, three severity degrees were distin guished: first degree of severity (the appearance of infiltrative changes) in 33.4 %, second degree (intensification of infiltrative changes) – in 46.6 %, complications (third degree of severity) occurred in 20 % of the patients.
Conclusions. The suggested method of XR assessing the severity of pneumonia in children with connective tissue dysplasia makes it possible to distinguish three severity degrees as well as to administer adequate therapy that will prevent development of severe forms of disease and complications.

SCIENTOMETRIC ANALYSIS OF CURRENT ISSUES OF TREATMENT OF NON-SMALL CELL LUNG CANCER

The purpose of the study. To make an assessment of current issues in treatment of non-small cell lung cancer due to  scientometric analysis of publications.
Materials and methods. The study was conucted by means of scientometric approach to publications processing. As a source of information, the abstrac resource Scopus was used. Its scientometric apparatus provides tracking of citation indicators of publications with retrospective since 1996. The search was carried out by means of the key term «non-small cell lung cancer». Data systematization was achieved owing to over time distribution of publications according to conuntries, paper type as well as profiles of authors, journals and establishments.
Outcomes and discussion. The search made it possible to find 53,378 publications of different types: scientific journals, collections of materials of scientific events (conferences, congresses, symposia, etc.), collections of scientific articles (Book series), Book (monograph), special (sectoral) edition in technical sciences, publications with information messages. Due to systematization of publications by titles of scientific journals, 158 items were found. The leader in the number of published articles is Lung Cancer Journal. Consistent interest in issues of treatment of NSCLC is observed in Journal оf Thoracic Oncology. The structure of the publications in subject area is indicative of the fact that not only medicine, but also biochemistry, genetics and molecular biology (33.0%), pharmacology (6.0%), chemistry (2.0%) and immunology (1.0%) are involved into solving of issues concerned with NSCLC treatment. Approximately 117 countries deal with elaboration of new techniques of NSCLC treatment where the top one is the USA followed by China, Japan, Italy, Germany, Britain and France. Within the scope of information flow under the study, the most frequently cited articles and authors such as Minna J. D., Komaki R., Smitt E. F., Nakagawa K. And others were specified. University of Texas Anderson Cancer Center turned out to be one of leading establishments.
Conclusions. The scientometric analysis of scientific publications over the period from 2000 to 2016, carried out due to the abstract resource of Scopus, showed a wide range of analytical possibilities and prospects for study of information flow of different scientific fields.

MULTIPLE METASTATIC DISEASES OF GENITALIA FEMALE, ARISING FROM PRIMARY GASTRIC CANCER

Abstract. Metastases to the female genital tract are observed in 6-28 % of patients with neoplasia of various localizations. In most cases they originate from gastric cancer and breast cancer. The article presents a clinical case of a patient with uterine cervical pathology, the diagnosis of which made it possible to detect stage IV gastric cancer with a latent clinical presentation. The article also contains data of various scientists dealing with clinical manifestations and incidence rate of this disease.
