ONCOLOGY AND RADIOLOGY IN LVIV: FROM THE FIRST STEPS TO ESTABLISHING

Summary. The latter half of the 19th century was marked by worldwide rapid development of science. A great number of discoveries were made in the Ukrainian scientific society, in Lviv in particular, including invention of new devices, introduction of hypotheses which in many cases became significant in terms of world civilization development. Invention activities affected all without distinction technical and natural sciences as well as medicine. This study is aimed at least to restore the names of our compatriots in memory, the genii who lent eclat to our nation. The discoveries made by Ivan Puliuy, inventions of Lvivian Jozeph Khan-Dembitskyi, Bronislav Sabat, Antoniy Tsieshynskiy and others should take a rightful place among the world scientific achievements.

RADIOLOGICAL METHOD IN ESTABLISHING OF SEVERITY DEGREE OF PNEUMOMEDIASTINUM IN PREMATURE INFANTS WITH LOW BODY WEIGHT WHEN APPLYING ARTIFICIAL LUNG VENTILATION

Objective. Improvement of diagnosis through objectification of severity degree of pneumomediastinum in premature infants with low body weight receiving artificial lung ventilation (ALv) by means of radiographic data analysis.

Materials and methods. The analysis of the data of chest X-ray (CXR) of 11 premature infants with low body weight receiving ALv, who were diagnosed with pneumomediastinum. Chest X-ray was carried out by means of РУМ-20М. To verify the diagnosis, all patients underwent ultrasound examination of the brain and the cardiovascular system by means of Doppler echocardiography together with complete clinical and laboratory examination. Results. The study is based on roentgenometric method of diagnosis of severity degree of pneumomediastinum in premature infants with low body weight receiving ALv and aimed at dimensioning of free air stripe under the mediastinal pleura, extension of the thymic shadow and downward displacement of the right cupula of the diaphragm. High correlation relationship between severity degrees and individual parameters as well as parameters by themselves has been proved. The analysis has made it possible to establish mild degree of pneumomediastinum in 45.5 % of patients, the second degree — in 36.4 % and the third degree — in 18.1 % of patients.

Conclusions. The suggested method of objectification of severity degree of pneumomediastinum according to roentgenometry data has made it possible to distinguish 3 degrees of the disease that is of great importance for treatment of children with low body weight.

PECULIARITIES OF BIOGENIC MONOAMINE METABOLISM AND DEAMINATION PROCESSES IN PATIENTS WITH GASTRIC CANCER

Purpose. Study of state of the sympathoadrenal and serotoninergic neurotransmitter systems in patients with gas-trocarcinogenesis and their predictive value for assessment of severity of the disease.

Materials and methods. The ergotropic and trophotropic body function state in 44 patients aged 43 to 68 years with gastric cancer has been studied. The research program included estimation of plasma epinephrine, norepinephrine, dihydroxyphenylalanine, dopamine, serotonin and platelet monoamine oxidase (MAO-B).

Results. Study of the sympathoadrenal system and biogenic amines deamination activity in patients with gastric adenocarcinoma showed decrease of content of dihydroxyphenylalanine, dopamine, norepinephrine, epinephrine in the blood plasma in the setting of increase of serotonin and platelet MAO activity, correspondently in II, III and IV stages of gastrocarcinogenesis development.

Сondusions. Development of carcinogenesis is accompanied by inhibition of DOPA, dopamine, norepinephrine, epinephrine synthesis and increase of serotonin synthesis as well as activity of oxidative deamination of catecholamines indicating reduction of ergotropic function and activation of trophotropic one.

APPLICATION OF INFRARED THERMAL IMAGING FOR MONITORING AND PREDICTION OF MUCOSITIS GRADE IN THE COURSE OF RADIOTHERAPY OF HEAD/NECK TUMORS

Introduction. Brief analysis of the problems associated with presence of radiation mucositis, its monitoring and early prediction was carried out.

Materials and methods. During radiation therapy, the quantitative changes in intensity of the abnormal thermal fields of patients with head/neck tumors were studied by means of thermal imaging methods.

Results. The coefficients of correlation R = (0.76-0.81) between thermal parameters and clinical grades of mucositis in patients during radiotherapy have been obtained. Changes in intensity of thermal fields detected 5-10 days before appearance of the first clinical symptoms of mucositis can be considered as an early prediction criterion of the individual grade of mucositis. Using the statistical analysis, the «good quality» (AUC = 0.79) of prediction based on the thermal criterion was obtained. It was noted that the sampling statistical power should be increased.

X-RAY FLUORESCENT ANALYSIS OF IODIN TRACES IN URINE

Purpose. Development of a new method of X-ray fluorescent analysis (XFA) for determination of iodine trace contents in urine in concentration range from 50 to200 gg/dm3 (0.05 - 0.2 ppm).

Materials and methods. Using XFA method, determination of iodine concentration in urine for 35 children of 10-15 with endocrine pathology (delay of sexual development, diffuse goiter, obesity) and 10 practically healthy children being observed under conditions of the consultative polyclinic and the department of endocrinology of SI «ISHCJ NAMSU»

Results. Optimization of XFA scheme has allowed lowering the detection limits for iodine in urine to 48 gg/dm3 and 67 gg/dm3 by fluorescence lines I-Lα and I-Lβ, respectively. Accumulation time was 3600 s. Satisfactory coincidence of the results for independent measurements by I-Lα and I-Lβ lines for each preparation on the iodine content in urine for patients with different pathologies makes it possible to recommend the X-ray fluorescent method for rapid diagnosis of urinary iodine.

Conclusions. The proposed optimized XFA method allows by 1-2 orders increasing detection sensitivity for micro-elements measurements in biology objects and attaining the iodine trace contents in urine in the range from 50 to 200 gg/dm3.

EXPERIENCE OF ORGANIZATION AND OUTPUTS OF WORK OF PET-CENTER IN STATE-FINANCED HEALTH INSTITUTION CHELYABINSK REGIONAL CLINICAL ONCOLOGY DISPENSARY

Summary. The paper presents experience of the first regional center of positron emission tomography in the Russian Federation. It also describes the results of the center for four years with the analysis of importance of performing of diagnostic procedures for cancer patients depending on the purpose of the study. The estimation of the information content of PET / CT in detection of lesions in comparison with other diagnostic methods has been carried out.

Summary. The article deals with the issue concerning epidemiology of pneumocystosis, clinical and radiological features of changes in the lungs in treatment of opportunistic infections. Indications for the use of computed tomography in HIV-infected and AIDS patients, radiation CT semiotics of changes in lung pneumocystosis, the possibility of establishing of exacted diagnosis in pneumocysosis of the lungs when using CT, differential diagnosis issues have been considered in the paper.

PECULIARITIES OF HYPOTHALAMIC-PITUITARY THYROID FUNCTION IN CHILDREN BORN FROM THE CHORNOBYL NPP ACCIDENT SURVIVIORS

Summary. The 168 children born to parents exposed after the Chernobyl accident were examined to study the hypothalamic-pituitary-thyroid system function in descendants of the Chernobyl survivors. Clinical, hormonal, ultrasound examinations and challenge test with tyroliberynom were conducted. Some abnormalities that might explain the functional strain of the hypothalamic-pituitary system were identified being of a great role in origination and progress of thyroid disease. The prolonged strain leads to decreased production of thyroid hormone, which causes an increased secretion of thyroid stimulating hormone. Under the influence of TSH the thyroid gland in the first stage becomes increased in its mass, which leads to the formation of endemic goiter. Stable and permanent thyroid gland enlargement often leads to formation of the nodular goiter and other proliferative processes, namely to carcinogenesis.

RADIONUCLIDE DIAGNOSIS OF DISEASE BILIARY SYSTEM OF CHILDREN

Summary. In the article the possibilities of dynamic Hepatobiliary scintigraphy is a radionuclide diagnostic for the detection and follow-up of patients with pathology of the biliary system. The degree of damage with biliary system is directly dependent on the degree of activity of the main process, its duration, the patient’s age and is independent of the treatment. An objective assessment of billiary system should be carried out to clarify the activity of the disease, the effectiveness of the treatment, the definition of subclinical signs of chronic and progression of the pathological process in the liver and gall bladder, the subsequent course of the disease prognosis in patients with liver disease of various origins.

OUTCOME ANALYSIS OF DINAMIC НEPATOBILIARY SCINTIGRAPHY AT CHILDREN BILIARY DYSKINESIA

Summary. In the article the possibilities of dynamic hepatobiliary scintigraphy is a radionuclide diagnostic for the detection and follow-up of patients with pathology of the liver and biliary system. According to our observations, it was found that the progression of the process in patients with pathology of the liver and biliary system are equally affected as the parenchyma of the liver and biliary ways, regardless of hyper-or hypokinetic state. An objective assessment of billiary system should be carried out to clarify the activity of the disease, the effectiveness of the treatment, the definition of subclinical signs of chronic and progression of the pathological process in the liver and gall bladder, the subsequent course of the disease prognosis in patients with liver disease of various origins.

EVALUATION OF LEFT VENTRICULAR REMODELLING IN PATIENTS WITH CHRONIC ISCHEMIC HEART DISEASE USING MULTISLICE COMPUTER TOMOGRAPHY AND MAGNETIC RESONANCE TOMOGRAPHY

Summary. The article presents the results of MSCT and MRI of the heart in 57 patients with chronic coronary heart disease. It determined the relationship between structural and functional changes in the left ventricle and the degree of left coronary artery stenosis. Also determined were the link between the ischemic left ventricular remodeling and depth of myocardial damage in patients with coronary heart disease and postinfarction cardiosclerosis.

MSCT and MRI are highly reliable imaging technique used to evaluate the infarcted and viable myocardium and postinfarct cardiac remodeling process.

RADIOIMMUNOLOGIC ANALYSIS (RIA) IN PROGNOSIS OF HEMABLOSTOSIS COURSE

Summary. The article is devoted to improving the prognosis of the AML, ALL and CHLL, assess the effectiveness and further monitoring of treatment for leukemia data based on the determination of TC and P2-MCG in blood serum by RIA. The results of studies of TC and P2-MCG in 220 adult patients with leukemia aged 17 to 78 years: 97 with AML, 36 with ALL, 87 CHLL. Provide a rationale for determining the appropriateness of including the TC and-P2-MCG complex surveys in patients with AML, ALL and CHLL as a reliable and independent method for prognosis of the leukemias and their complications. The values of initial TC and P2-MCG patients may be divided into prognostic groups to establish the probable duration of survival, monitoring of treatment. For patients with AL TC values at more than 20.0 U/l in patients with CHLL — 30.0 U/l and at a level of P2-MCG exceeding 10.0 mg/l in all cases. Proved that patients with AML, ALL and CHLL had the highest survival in achieving a complete remission with a minimum initial concentrations of TC to 10.0 U/l and P2-MCG to 5.0 mg/l. On the dynamics of the content of TC and P2-MCG in remission may anticipate the threat of recurrence and timely use of appropriate medical tactics: with increasing levels of TC AML more than 6.0 U/l for ALL - 9.0 U/l for CHLL - 20.0 U/l.

PROTECTIVE EFFECTS OF PACLITAXEL ON Y-IRRADIATION ACTION ON ANAPLASTIC THYROID CANCER CELLS

Summary. The aim of the paper was to describe the effects of Paclitaxel (Ptx), y-irradiation (IR) and their combination in undifferentiated thyroid cancer cells (ATC). The Ptx at low concentrations caused noticeable radioprotective effect. The combined effect of radiation and Ptx enhanced antiapoptotic Bcl-2 phosphorylation, and survivin expression. The effect of these events can be considered as antiapoptotic — Ptx attenuated cytotoxic effect of IR.

THE FEATURES OF KINETICS OF 153SM-OKSABIFOR IN BONE METASTASES OF CANCER OF DIFFERENT LOCALIZATION

Summary. Provide evaluation of investigation of kinetics of 153Sm-oksabifor in metastases of cancer the results of complex scintigraphic investigations of 26 patients who were treated by radionuclide therapy were analyzed. The angioscintigraphy during 60 seconds, dynamic scintigraphy during 60-90 minutes and whole body scanning after intravenous bolus injection of 4130-4950 MBq of the radiopharmaceutical in sequence were made. Considerable variability of indices of the radiopharmaceutical kinetics was determined. The features of angioscintigrams from bone metastases with intensive accumulation of the radiopharmaceutical are a short descending segment or its absents and passing an ascending segment into plateau or slowly ascending curve, which are determined the radiopharmaceutical fixation in metastases during first passing the bolus through the vessels. Character of the radiopharmaceutical fixation at angioscintigraphy may be a prognostic factor of the efficacy of radionuclide therapy.

THE CYTOLOGICAL AND IMUNOCYTOCHEMICAL ADVANCES OF THE PRIMARY THYROID PAPILLARY CARCINOMAS AND THEIR METASTASES (RADIOIODINE RESISTANT AND RADIOIODINE UPTAKE)

Summary. The cytomorphological characteristics of the primary thyroid papillary carcinomas and their metasta-ses have no significant differences. In the same time, the radioiodine resistant metastases and radioiodine uptake metastases have different cytomorphological characteristics. The cytomorphological characteristics of the cells and the expression of the thyroid peroxidise in the metastases reflects cell’s differentiation and their ability to accumulate of radioiodine. We’ve not found difference in the expression of the cell’s proliferative activity antigen — Кі-67 between radioiodine resistant metastases and common cohort of papillary carcinomas.

STATE OF PITUITARY-GONADAL SYSTEM AND MORPHOFUNCTIONAL INDICES OF SPERM IN MALES WITH THYROID CANCER AT DIFFERENT STAGES OF TREATMENT

Summary. The authors have followed up 21 patients with thyroid cancer treated with radioactive iodine. The data obtained suggested the development of transient dysfunctions of pituitary-gonadal system in males with thyroid cancer in part of patients, with signs of subclinical androgen deficiency. An examination to patients having received a first course of radioiodine therapy showed different variants of abnormalities of morphofunctional indices of the sperm.

Further investigations on a larger contingent of patients are necessary to clarify the impact of radioiodine therapy on the morphofunctional state of pituitary-gonadal system and reproductive function in males with thyroid cancer.

COMPARISON OF ULTRASOUND AND SONTIGRAPHY OF THE PARATHYROID GLANDS DIAGNOSTICS OF THE FOCAL PATHOLOGY. THE RESULTS OF OWN RESEARCH

Summary. Primary hyperparathyroidism (PHP) is the most common cause of hypercalcemia in ambulatory patients. For the diagnosis of this disease as usually use scintyigraphy of parathyroid glands (PTSG) with 99m-MIBI Ta and ultrasonography (US). 87 patients with PHP were underwent to PTSG and 80 patients — to ultrasound of the neck area. 76 from 87 patients had the patological focuses of parathyroid tissue on PTSG, 11 had not the signs of the disease. Sensitivity PTSG in diagnosis of focal lesions was 90.54 % an accuracy — 81.61 %. 51 from 80 patients had patological focuses of the parathyroid glands on US and 29 patients had not signs of the disease. Sensitivity of US in diagnosis of focal lesions was 64.29 %, accuracy — 61.25 %.

PTSG more informative in identifying diseased parathyroid tissue as compared to ultrasound investigation.

EXPERIENCE WITH 99mTC-TEKTROTYD IN THE DIAGNOSIS OF ECTOPIC LOCALIZATION OF ACTH-SECRETING TUMORS IN PATIENTS WITH CUSHING’S SYNDROME

Summary. The differential diagnosis of Cushing’s disease is often carried out with Cushing’s syndrome, caused by the presence of tumors producing bioactive ACTH or ACTH-like substance. The aim of the study was to determine the effectiveness of the use of scintigraphy with 99mTc-Tektrotyd in the diagnosis of ectopic localization of ACTH-secreting tumors in patients with Cushing’s syndrome. The survey of 25 patients with elevated levels of ACTH in the peripheral blood, allowed in 10 (40 %) patients identify foci of increased uptake radiopharmaceutical analog of somatostatin . It was found that the scintigraphy 99mTc-Tektrotyd is sensitive and specific method in determining the foci of ectopic ACTH production. The use of scintigraphy with 99mTc-Tektrotyd may be a good alternative to studies with octreotide labeled with 1UIn or 123I in the diagnosis of ectopic ACTH syndrome due to lower radiation dose to the patient, the higher picture quality and greater availability of this radiopharmaceutical.

COMPLEX RADIONUCLIDE EVALUATION OF STRUCTURAL AND FUNCTIONAL KIDNEYS DISORDERS OF CHILDREN WITH CHRONIC OBSTRUCTIVE PYELONEPHRITIS

Summary. Children's chronic obstructive pyelonephritis (COPN) is the most important and difficult among microbes-inflammatory kidney's deceases. Kidney's COPN functional disorders are evaluated generally by clinical and laboratory criteria which not always have high informativity. Scintigraphic kidney's examinations with nephrothropic radiopharmaceuticals (NRP) are more informative.

Conclusion. The most informative are 99mТс-ЕС and 99mTc-DTPA for initial examinations, 99mTc-DTPA and 99mTc-pyrophosphate for monitoring examinations.

APPLICATION OF SPECT AND SPECT/CT IN DIAGNOSIS OF ATYPICAL LOCATED ADENOMAS OF THE PARATHYROID GLANDS

Summary. The aim of our study was to explore the possibilities and the effectiveness of SPECT and SPECT/ CT technologies with 99mTc-MIBI in diagnosis of atypical located adenomas of the parathyroid glands. The study involved 143 patients with tumor pathology of the parathyroid glands. In 115 patients (80,4 %) high-quality diagnostic images were obtained. Negative results of scintigraphy occurred in 16 patients (11,2 %). Questionable results of scintigraphy occurred in 9 patients (6,3 %). The authors conclude about high diagnostic value of scintigraphy with 99mTc-MIBI in diagnosis of tumor pathology of the parathyroid glands and SPECT/CT with 99mTc-MIBI in diagnosis of atypical located adenomas of the parathyroid glands.

COMPARISON THE PLANAR BONE SCYNTIGRAPHY AND SPECT BY BENIGN AND MALIGNANT LESIONS OF MUSCULOSCELETAL APPARATUS IN PATIENTS WITH ONCOLOGICAL DISEASES

Summary. The aim of our study was to compare abilities of planar BS and SPECT for differential diagnostic skeletal lesions in patients with oncological diseases.

It was revealed that SPECT allows to reveal more pathological lesions, to improve their differential diagnostic, and to decrease the amount of equivocal lesions in patients with metastatic skeletal injuries.

Areas of slight intensity of RPH fixation can have metastatic origin, which can be confirmed by SPECT.

STUDING THE FACTORS OF RECURRING THYROID CANCER

Summary. From 250 patients with the differentiated thyroid cancer (DTC), who was treated at our hospital or pass monitoring, was selected 139 patients. Analysis clinical especially of DTC recurrences was specified. It was identified that almost in the identical amount of cases the recurrences of DTC can had both local and/or distant character. Dependence between the emergence of DTC recurrences and primary prevalence of tumor process is established. We concluded that the origin of recurrences DTC depends of previous surgical treatment and independent from the volume and total ablative activity of radioiodine therapy.

CLINICAL AND ECONOMIC ANALYSIS OF METHODS OF DIAGNOSIS OF DIFFERENTIATED THYROID CANCER RELAPSES

Summary. A pharmacoeconomic analysis of methods for detecting recurrences and / or metastases of differentiated thyroid cancer was performed and a comparative evaluation of the economic feasibility of the two strategies of relapse diagnosis of 45 patients was carried out.

Patients with elevated thyroglobulin more than 2.1 ng / ml are consistently performed neck ultrasound, scintigraphy with 99 mTc(V)-DMSA, computed tomography of the chest and scintigraphy with medical activities 131I. Reduction in the cost of diagnosis of recurrence and / or metastasis of DTC with using the proposed scheme is due to the fact that for the detection of recurrence and / or metastases the method of scintigraphy 99 mTc(V)-DMSA of much less value is used, and the need to use one the most expensive of all the diagnostic tests — scintigraphy with 131I occurs for a small number of (few) patients.

Thus, the administration of study with 99 mTc(V)-DMSA into a diagnostic algorithm of relapse detection of patients with DTC reduces the value of the diagnostic complex by 18 %.

WORLD EXPERIENCE OF POSTGRADUATE TRAINING OF DIAGNOSTIC RADIOLOGISTS IN RESIDENCY

Summary. The purpose of our study was to compare various aspects of radiology training schemes in 10 countries. A questionnaire was sent to senior residents in the UK, the USA, Canada, Australia, New Zealand, Italy, Egypt, India, Malaysia and Greece. The questions concerned length of training, pre-training experience, organization of the training scheme, teaching, resources, independent work, examinations, fellowship and progression to jobs. A wide variety of training programs, self-learning, examination and assessments occurs across the world. In view of the «internatiolization» of radiology services and the variation in training style an international qualification for quality assurance purposes may be very complicated.
