Morphological picture of skin radiation lesion development in rats at spontaneous healing and at phototherapy

Objective: To investigate the effect of photomatrix therapy with monochromic light of the optic range on the morphofunctional picture of radiation lesion healing in rats after local irradiation.
Material and Methods: The study was performed on 28 adult male Wistar rats weighing 180-200g with single local x-ray exposure (LE) of the right thigh area.
X-ray exposure at a dose of 80.0 Gy was performed with a radiotherapy unit TUR-60. Photon treatment of the irradiated area started on the day of exposure. The course of photon therapy (PT) lasted 5 days (3 treatments 20 minutes each during the day). Photon matrix unit "Barva-Fleks" with matrices of super-bright red and blue light diodes was used. All animals were divided into 3 groups: controls (rats with LE), and 2 study groups consisting of rats with LE and PT with red or blue light. The tissue for morphology investigation was taken on days 3, 7, 14, 21, 30 and 60 after the exposure. The tissue from the area of local exposure or radiation ulcer was fixed in 10% formalin and after paraffin treatment the slices were stained with hematoxylin-eosin according to Van Gieson.
Results: The analysis of the morphology findings of the skin radiation lesion (SRL) at PT with red or blue light allowed to reveal some differences in their action. In early (days 7-14) period of local radiation lesion development PT did not promote considerable therapeutic effect but created conditions for further reparative processes in the focus of radiation lesion. Blue light stimulated local immune response to inflammation earlier, which was observed on day 7. Treatment with blue light diminished the size of superficial necrosis much earlier (on day 21) than in the controls and in the "red" group. Necrosis organization processes in the deep layers of the skin developed more rapidly in the "blue" group, while when treating with red light, small necroses and immature granulation tissue persisted in these layers for along time.
Conclusion: In rats with local RSL which were delivered phototherapy epithelization and complete fibrotization of all skin layers of the wound were over on day 30, while in the controls epithelization and partial fibrotization of radiation ulcer occurred only on the second month of the observation. At application of blue light, the signs of fibronogenesis activation and reparation in the zone of radiation lesion were observed before day 30. Therapeutic effect of blue light dominated over red light due to earlier stimulation of cellular and immune reactions in the derma.

Neurohumoral regulation of endothelial function in adolescents with myocardial pathology

Summary. The purpose of the work was to investigate the influence of the systems of neurohumoral regulation and immune inflammation activation on the function of vascular endothelium in children with myocardial pathology. The study involved 63 teenagers with the history of myocarditis and syndrome of heart connective tissue dyspepsia. The endothelial function was assessed using the test with reactive hyperemia and nitroglycerin. Circadian excretion of catecholamines with urine, activity of plasma renin, angiotensin-II, blood levels of aldosterone, endothelin-1, prostacyclin, thromboxane, interleukin-1p, -6, TNF-a were investigated. It was established that increase of the artery diameter in the test with reactive hyperemia was on an average 8.72 %, which was significantly lower than in healthy children (p < 0.05). In 57.14 % children with myocardium pathology, endothelium-dependent vasodilatation was insufficient. Endothelial dysfunction was observed against a background of low levels of catecholamine excretion in the urine and aldosterone amount in the blood, increased endothelin-1 plasma level and blood Il-6.

The peculiarities of cortisone production in type 1 diabetes mellitus in children and adolescents according to radionuclide findings

Summary. The character of cortisone production was investigated in children and adolescents with type 1 diabetes mellitus. Cortisone production was detrained three times a day using radioimmune technique with standard commercially available kits «HOPIBOX» (Belarus) in 192 children and adolescents aged 3-18 with DM. It was established that cortisone level in children and adolescents with type 1 DM exceeded the reference values irrespective of the disease duration and the state of carbohydrate metabolism compensation. In girls, hypercortisolemia was revealed more frequently than in boys, especially in puberty (stage II-III according to Tanner).

Static liver scan in assessment of liver function in adolescents with obesity

Summary. The functional state of the liver was assessed in adolescents with obesity using static liver scan. It was established that static liver scan in adolescents with obesity allowed to reveal diffuse liver lesions suggesting latent forms of hepatobiliary system lesions, which promoted specification of the diagnosis and timely treatment.

The state of renin-agiotensin-II system and morphofunctional peculiarities of the heart in girls with oligomenorrhea

Summary. One hundred twenty girls with oligomenorrhea were examined with the purpose to assess the activity of renin-angiotensin-II-aldosteron system (RAAS) and morphofunctional peculiarities of the heart. Echocardiography and Doppler ultrasonography indices of the heart were analyzed. RAAS activity was assessed. Disorders of cardiohemaodynamics, prevalence of hypokinetic type of hemocirculation in combination with multiple minor heart anomalies promoting development of dysplastic cardiomyopathy were revealed. It was revealed that morphofunctional disorders in girls developed against a background of increased angiotensin-II and aldosterone levels.

The state of immune system in thyroid cancer with metastases to the lungs after radioiodine therapy

Summary. The purpose of the work was to investigate the influence of radioiodine therapy on immunological indices of the patients with thyroid cancer and metastases to the lungs. Phenotypic profile of the cells of peripheral blood was determined in 36 donors and 82 patients with differentiated thyroid carcinoma (46 with metastases to the lungs, 36 without distant metastases) before and on day 6 after radioiodine therapy. Significant changes of lymphocyte phenotype in the blood were revealed in early terms after administration of therapeutic activities of 131I (4130-4 730 MBq): decreased amount of CD19+, CD3+ and activated HLA-DR+-cells, disbalance of T-lymphocyte population (CD4+, CD8+), increase of CD95+-cells. In patients with lung metastases, the degree of immune disorders was significantly higher. Radioiodine therapy caused the changes in peripheral blood lymphocyte population and expression of markers of activation and apoptosis. Great radiation load on the blood cells can increase the disorders in the immune system of the patients with diffuse lung metastases.

Cytomorphological peculiarities of radioiodine-resistant metastases of papillary thyroid cancer

Summary. Cytomorphological study of the punctates from radioiodine-resistant metastases of papillary carcinoma (PC) of the thyroid gland (TG) obtained from 35 patients was performed. Cytomorphological picture of these punctates did not differ significantly from total PC population. Specific morphological structures, occurring in the punctates of radioiodine-resistant (RIR) metastases 2.6 times more frequently than in the general population of TG PC, were revealed. Comparative analysis of pathohistological conclusions of primary tumors and RIR metastases of TG PC was performed. It was shown that presence of necrotic changes in the primary tumor can be a prognostic factor of radioiodine resistance, while loss of follicular structures by RIR metastasis when compared with an ordinary tumor is a pathomorphological but not a cytomorphological manifestation of radioiodine resistance.

The peculiarities of oxidation homeostasis in boys and girls born in the families of participants of Chornobyl accident clean-up

Summary. The state of lipid peroxidation (LP) and antioxidant system (AS) was investigated in boys and girls aged 16-18 born in the families of participants of Chornobyl accident clean-up.
In the boys born from fathers who participated in Chornobyl accident clean-up, inhibition of LP processes and activity of superoxide dismutase (SODM) in the blood serum as well as increased activity of glutathione peroxidase in the erythrocytes were more frequent than in girls. In the majority of girls, increased SODM activity both in the blood serum and erythrocytes was significantly common. The revealed disorders suggest about regulation disbalance of the key antioxidant enzymes in boys and girls at the end stage of the childhood.

The peculiarities of leptinemia in adolescent girls with menstrual function disorders depending on insulin-resistance presence

Summary. The purpose of the work was to investigate the changes in leptin level in girls with menstrual function disorders (MFD) depending on insulin resistance (IR).
Association of blood serum leptin level changes and IR was revealed in patients with oligomenorrhea (OM) and secondary amenorrhea (SA). In half of the girls with pubertate uterine bleedings irrespective of IR, hypoleptinemia was revealed, which occurred more frequently than in OM and SA. One third of the patients with MFD demonstrated combination of leptin resistance with IR, which is a risk factor of polycystic ovary syndrome formation and metabolic syndrome.

Disorders of thyroid function in forming insulin resistance of the children from the parents irradiated due to Chornobyl accident

Summary. The purpose of the research was to determine thyroid function disorders and their influence on forming insulin resistance (IR) in children from parents exposed to radiation due to Chornobyl accident. The study involved 108 children aged 12-14. The findings demonstrated latent hypothyroidism in children with thyroid pathology without clinical signs of thyroid dysfunction against a background of metabolic changes. This proves the authors’ suggestion that reduction of thyroid function is an important factor of IR formation, which, in turn, promotes development of peripheral resistance to thyroid hormones.

The role and diagnostic significance of dynamic liver scan in patients with chronic cholecystitis, bile stones and gallbladder dyskinesia

Summary. Forty-six patients aged 29-63 were investigated with the purpose to study the role of dynamic liver scan with 99mTc-Mesid in differential diagnosis of functional changes at chronic cholecystitis, bile stones, and gallbladder dyskinesia. Dynamic liver scan was performed in 5 patients with cholelithiasis, 5 with afunctional gallbladder, 19 with chronic cholecystitis and 17 with bile duct dyskinesia. Absorption-excretion liver function and concentration motor function of the gallbladder, bile duct permeability were assessed. It was established that in contrast to structural diagnostic methods (US, CT, MRI) assessing morphological changes in the tissues, dynamic liver scan allows to investigate functional changes in the hepatobiliary system (concentration, excretory function of the gallbladder and absorbing liver cells).

Summary. The capabilities of bone scan in orthopedic and traumatology practice are featured. The literature data about the use of planar bone scan, single photon emission computed tomography (SPECT) and combination of SPECT/CT in the lesions of bones and joints of various origin (injuries, infections, primary and secondary malignancies, assessment of post-operative and transplantation bones and joints) are reported.
The indications to the use of different modes of bone scan, their peculiarities and advantages over the other methods of radiation diagnosis in definite clinical cases are shown.
The authors conclude about the prospects of bone scan use for diagnosis, determining the stage and dissemination of the pathological process in the skeleton as well as control of the treatment efficacy.

Uterus and ovaries ultrasonography and androgenicity index in women with differentiated thyroid carcinoma after surgery against a background of radioiodine therapy

Summary. The authors report the findings of US monitoring of the structure of the uterus and ovaries as well as the results of investigation of sex steroid binding globulin (SSG) in 78 patients in the dynamics of the menstrual cycle during 1-year observation after thyroidectomy and radioiodine therapy for differentiated thyroid carcinoma (DTC). It was established that beginning from the post-operative period they demonstrated US signs of anovulatory cycle and lutein phase deficiency. It was established that the state of pronounced hypothyroidism due to thyroidectomy was accompanied by marked reduction of SSG and SSG/testosterone coefficient in the blood when compared to the pre-operative indices, which suggested about hyperandrogeny development in the investigated women. Increased androgenicity index in women operated for TC can promote disorders in the cyclic ovary function, which is registered during the first year after thyroidectomy and radioiodine therapy.

The role of dynamic hepatobiliscintigraphy in diagnosis of Oddi’s sphincter functionalstate

Summary. Dynamic hepatobiliscintigraphy was proposed to diagnose the functional state of Oddi’s sphincter, namely to assess the duration of Oddi’s sphincter latent period. The study involved 50 patients with hepatobiliary diseases, of them 4 after cholecystectomy. The normal duration of the latent period was 5.28 ± 0.26 min; in patients with hypotonic dysfunction it lasted 1± 0.06 min; those with hypertonic Oddi’s sphincter 10.77 ± 0.53 min. The method also allows to reveal the disorders of the motor activity of the pancreatobiliary anastomosis and to establish the accompanying disorders of the liver and gallbladder function, which is aimed at the treatment optimizing.

Correlation of insulin and sex steroid binding globulin level in teen-age girls with menstrual function disorders

Summary. The purpose of the work was to investigate the correlation between the levels of sex steroid binding globulin (SSBG), insulin and a number of indices of lipoprotein blood spectrum in teen-age girls with menstrual function disorders (MFD).
The study involved 247 girls aged 13-17 with MFD. Blood serum insulin, SSBG, total cholesterol, high density lipoproteins, triglycerides amount was investigated.
The results of the study demonstrate high incidence of insulin resistance (IR) in girls with MFD. In contrast to adult women with IR, in teen-age girls with MFD, IR is not always accompanied by reduction of SSBG level and the decreased SSBG level by changes in the lipoprotein blood spectrum of atherogenic trend.

The role of thyroglobulin level assessment in diagnosis of relapses and metastases ofdifferentiated thyroid cancer

Summary. Differentiated thyroid cancer is the most common tumor of the endocrine system. In the recent decade significant success has been achieved in complete treatment of the disease but the question of monitoring of the patients who underwent treatment is still urgent. The study involved 36 patients with thyroid ablation and increased thyroglobulin level. To establish the causes of increased thyroglobulin level echosonography of the neck and liver, chest x-ray and, when indicated, computed tomography were performed. Relapses and/or metastases were revealed in half of the patients. In 13 cases these findings were proven by other methods of investigation (in 8 patients by ultrasonography with cytology verification and in 5 patients with computed tomography). In 5 patients with increasing thyroglobulin level relapses and/or metastases of thyroid cancer were established with scintigraphy on the post-treatment scans. Eighteen patients more are waiting for radioiodine treatment followed by post-treatment scan. Increased thymoglobulin level in patients with thyroid ablation should be considered a sign of relapses or metastases of threefold cancer.

ß2-microglobulin as a prognostic factor at chronic lymphocytic leukemia

Summary. The findings of determining b2-MG level in the blood serum of 87 patients with B-cell chronic lymphocytic leukemia (BCCLL) during the treatment using standard protocol FC (Fludarabine + Cyclophosphamide) or FC-R (Fludarabine + Cyclophosphamide + Rituximab) are presented. It was proven that blood serum b2-MG is an independent marker of the malignant course and the rate of BCCLL progress. It is also a prognostic criterion of response to CT and treatment efficacy. The lower is b2-MG level at the moment of the disease diagnosis; the higher is the probability of remission. Reduction of b2-MG after CT completion to its normal values, as a rule, corresponds to the presence of remission. Absence of its normal concentration after the end of the treatment predicts further progress of the disease in the nearest future.

The effectiveness of single photon emission computed tomography in prognosis of malignant brain gliomas grade

Summary. Retrospective analysis of the observations demonstrating discrepancy between emission tomography and pathomorphology findings in diagnosis of brain gliomas was made. The causes of this discrepancy were investigated.
The study involved 39 observations of brain gliomas in which the findings of single photon emission computed tomography contradicted the findings of histology investigations in determining the degree of malignancy of these tumors.
Profound histological analysis showed that visualization of glial tumors with SPECT suggested about its increased vasculaization and, consequently, a high degree of malignancy. Absence of glioma image on SPECT suggestsed of decreased blood supply and low degree of malignancy. Sometimes discrepancies between SPECT and histology findings can be due to insufficient amount of surgical material delivered for histology diagnosis.
Thus, SPECT allows predicting the degree of malignancy of brain gliomas in 99.5 % of patients. Discrepancy between SPECT and histology findings are observed in 0.5 % of patients, which can be explained by the peculiarities of their histological structure or insufficient amount of surgical material delivered for histology investigation.

The state of thyroid gland and iodine supply in the members of Ukrainian-American thyroid project residing in Chernigiv Region

Summary. The study involved 1499 children residing in Repkinskiy and Chernigivskiy Districts of Chernigiv region who were included in the group of Ukrainian-American thyroid proj ect. The state of the thyroid gland was assessed using ultrasonography, thyroid hormone amount and TTH in the blood, antibodies to thyroid gland antigens, iodine supply according to daily ioduria.
The findings of the research demonstrated a high incidence of thyroid pathology: 18 malignant tumors, 7 follicular ademomas, 844 non-toxic goiters, 2 toxic goiters.
The most common pathology (about 30% of the investigated patients was diffuse non-toxic goiter. Its main cause was shown to be insufficient consumption of iodine (iodineuria median ranged within medium degree of iodine insufficiency severity). These findings were proven in a considerable number of the patients by increased level of thyroglobulin and TTH.

Experimental assessment of changes in 99mTc-MIBI kinetics in Walker-256 carcinoma under the influence of spatially uneven electromagnetic

Summary. Magniterm unit (Radmir, Ukraine) was used with the purpose to assess the changes in 99mTc-MIBI kinetics in noninbred female rats with transplanted Walker-256 carcinoma after electromagnetic irradiation (EI) of the tumor in spatially uneven (SU) field. The applicators creating relatively even and SU field were used for EI. Scintigraphy was done using gamma-camera ГКС301Т. It was established that moderate electromagnetic hyperemia increased effective tumor blood flow, promoted increase of capture intensity and reduction of wash-out of 99mTc-MIBI from the tumor cells. The effect of SU electromagnetic field was more pronounced. Moderate electromagnetic hyperemia not causing considerable heating of the tissues can be used to expand the possibilities of radionuclide diagnosis.

The capabilities of up-to-date SPECT in early post-operative diagnosis of intracerebral malignant tumors remnants

Summary. To determine the capabilities of SPECT in early postoperative diagnosis of tumor remnants in patients with malignant brain tumors, this method was used in 14 patients with suspected focal brains lesions together with other methods of neuroimaging (MRI and CT) before and after surgery. The diagnosis was verified histologically in all cases.
The analysis of the findings showed that focal hyperfixation of the RP was noted in all tumors. In early post-operative period SPECT signs of pathological hyperfixation of the RP in the areas of the tumor were absent in 13 patients. Only in one case, postoperative emission tomograms (in contrast to CT) demonstrated an area of the RP hyperfixation, which corresponded to the remnants of the tumor tissue.
The study showed that SPECT is effective in diagnosis of intracerebral tumors and can be used as an alternative method before and after the surgery. In some cases SPECT can be more informative than CT in diagnosis of malignant gliomas remnant tissue.

Investigation of blood serum lymphokine and hormone level in men with type 2 diabetes mellitus and metabolic syndrome

Summary. The findings of the investigation demonstrated that in patients with type 2 diabetes mellitus (DM) with metabolic syndrome and without it as well as in the group with MS without DM TNF-б and IL-6 levels were increased, while cortisol, aldosterone, ACTU and insulin levels were decreased. Insulin level as well as that of ACTH was directly proportional to the body mass index. The performed investigation revealed androgen deficiency in all groups of the investigated patients. A tendency to increased estradiol level was revealed in men with metabolic syndrome in case of type 2 DM development.

Radionuclide methods in assessment efficacy of drug treatment for diabetic osteoarthropathy and angiopathy of the lower extremities

Summary. The capabilities of radionuclide angiography and bone scan were determined in diagnosis of diabetic angiopathy and arthropathy of the lower extremities in patients with diabetes mellitus (DM). The study involved 88 patients with DM and angiopathy and osteoarthropathy of the lower extremities of diabetic origin, of them 42 patients with grade II microand macroagiopathy and foot ulcers, 36 patients with knee bone involvement, 10 patients with DM and diabetic foot who were examined before the treatment, 1. 5 months and 1. 5 years after the treatment with Wobenzym. In patients with DM and diabetic foot, hemodynamic disorders manifesting by blood flow delay in the large and medium-sized vessels, arterioles and capillaries were revealed. Bone scan allowed to determine inflammatory processes in the foot and knees. Antiinflammatory effect of Wobenzym was demonstrated in patients with DM with diabetic foot, but its influence on homodynamic in the foot vessels was lesss pronounced.

Assuagement of left ventricle diastolic function using radionuclide ventriculography in patients with coronary artery disease

Summary. A complex of indices of left ventricle diastolic function was worked out using the findings of radionuclide ventriculography in patients with coronary artery disease (CAD). The complex includes peak filling rate (FRмакс, с-1), filling fraction of rapid filling phase (FP1), characterizing the state of active energy-dependent process of LV contractile myocardium relaxation, phase of slow filling (FP2). Comparison of FP1 and FP2 allowed to determine homodynamic structure of LV filling and the character of its changes in patients with CAD.
An index of volume compliance as a ratio of LV filling volume in the phase of rapid filling to the volume of LV filling in the phase of slow filling (V1/V2), reflecting the state of viscous-elastic properties of the LV wall, compliance of the LV chamber was suggested.
The patients with CAD demonstrated the changes of active energy-dependent process of LV relaxation, changes of hemodynamic structure of LV filling with blood and reduction of LV compliance increasing with aggravation of the LV systolic function and more pronounced in patients with post-infarction cardiosclerosis.

131I-MIBG in diagnosis and therapy of medullar thyroid cancer (the state of the problem)

Summary. The authors analyze the literature about the problem of diagnosis and treatment of medullar thyroid cancer and highlight the findings of the investigation on the role of radionuclide methods in diagnosis of metastases and relapses of medullar carcinoma, choice of radiopharmaceuticals for systemic radionuclide therapy, compare them using the data of multi-center studies. The role of 131I-MIBG in treatment of medullar thyroid carcinoma was determined.

Capabilities of complex scintigraphy in investigation of cystourethral reflux in children

Summary. The authors describe the capabilities of radionuclide methods in timely diagnosis and dynamic observation of cystourethral reflux in children. It was proven that indirect radionuclide cystography with standard dynamic renoscintigraphy could promote an important diagnostic complex of data for revealing and observation of CUR. Complex scintigraphic investigation can be recommended for revealing and dynamic observation of the disease course which allows to improve the quality of diagnosis and reduce radiation load to the children.

Development of technologies and organization of radiopharmaceuticals production at Institute of Nuclear Research of National Academy of Science of Ukraine

Summary. Technologies of radiopharmaceuticals (RP) production based on reactor radionuclides of 99Me (99mTc) and 131I: solution of sodium pertechnetate-99mTc from centralized extraction generator of research reactor ВВР-М; portable helium Мо-Zr generators 99Mо/99mTc; solution of sodium iodide (131I) were worked out at Institute of Nuclear research of National Academy of Science of Ukraine. A technique of production of sodium iodide (131I) in capsules was prepared, which can allow working out a technology for industrial production. For technetium generators,99Mо is obtained at irradiation of heat neutrons of the reactor of natural molybdenum oxide. To obtain 131I, metallic tellurium is irradiated. One-stage technology of 131I isolation from metallic tellurium using thermographic method was worked out. Physical-chemical parameters of the RP meet the requirements of European Pharmacopeia.

Determining thyroxin binding globulin amount in adolescents with diffuse non-toxic goiter using radioimmune methods

Summary. The purpose of the work was to determine the character of thyroxin binding globulin (TBG) production in adolescents with diffuse non-toxic goiter (DNTG). Blood serum TBG, TTH, free fractions of thyroxin and triodothyronine, total fractions of testosterone and estradiol levels were investigated in 140 teen-agers of both genders aged 19-16 with DNTG by radioimmune method using Immunotech kits (Czech Republic). The obtained findings suggest that TBG level is increased in girls with DNTG in the period of early puberty, which can be promoted by increased estradiol production. It was proven that increased TBG production was combined with increased TTH level. In boys with DNTG, TBG level in the period of early puberty was significantly lower than in girls and was not associated with TTH indices.

The experience of 1-131 application in treatment of diffuse toxic goiter

Summary. The article deals with the analysis of iodine-131 therapy efficacy in patients with diffuse toxic goiter (DTG). Considerable improvement of the state of the patients, including those with ophthalmological signs, as well as absence of side-effects are shown. The literature data and the original experience allow to recommend iodine-therapy for wider use in treatment of this disease.

Current control of output parameters of digital x-ray equipment as an important component of x-ray examination quality assurance

Summary. The paper defines the conditions of viability and effectiveness of the system to control digital x-ray equipment. It is shown that it is unreal to create a control system, similar to existing in European countries in the absence of regulations, qualified engineering personnel and financial resources for maintenance of this equipment. A possibility of gradual introduction to radiology practice of current control of digital image quality with test phantoms and periodic monitoring of equivalent dose in the plane of the digital detector to estimate radiation exposure to the patient is justified.

The role of digital technologies in reduction of patient radiation loads during x-ray examinations

Summary. The paper discusses the ways to reduce radiation exposure on patients using digital technology of x-ray images at chest x-ray, radiography and fluoroscopy. It is shown that transition to digital technologies in radiology can potentially decrease 2 times the collective effective dose on the Ukrainian population. The important role of x-ray department staff training of appropriate use of new technologies to realize their potential benefits in practice is shown.

A system of recording the equipment and personnel of x-ray units of a region

Summary. The authors propose to create a uniform information base “Units File” which can be uploaded to the Internet and renovated once in three months. This base can allow a specialist of any level and organizations to obtain the following information the name of the region, the name of the medical establishment, the number of units, availability of working units, their version or type, year of production, the state of dosimetry control in the unit, the number and state of the means, etc. The authors conclude that it can be of considerable use and cost effective.

Hygienic issues of preventive state sanitary and epidemiology surveillance of location of hospitals with x-ray units

Summary. The work emphasizes that medical establishments with x-ray units are an object of state sanitary and epidemiology surveillance. The authors stress that according to the respective regulatory documents, radiology departments having a separate project should be placed in separate buildings or isolated departments of medical establishments. According to the requirements of sanitary rules and norms 6.6.3-150-2007, it is necessary to obtain permission for stationary protection from sources of ionizingradiation, as well as to have passports for the sources and the right to work with them. Location of medical establishments in residential buildings is possible when the Norms of Radiating Safety of Ukraine are observed under the control of the specialists of Institute of Hygiene and Medical Ecology named after O.M. Marzeyev.

Main problems of state sanitary-epidemiological surveillance of ionizing radiation sources application in medical practice

Summary. It is emphasized that main contribution to medical exposure of the country is made by mass-scale x-ray investigations. About 21 mln investigations are performed annually, of them 1.8 mln preventive chest x-ray examinations, 1.3 mln fluoroscopies, 552 thousand radiologic investigations; about 93 thousand patients are delivered radiation therapy. Ministry of Health of Ukraine pays special attention to licensing of the quality of medical services as well as radiation protection of the personal and patients and availability of up-to-date equipment. The authors conclude that due to increased use of ionizing radiation sources it is necessary to correct and revise the current sanitary rules and norms.

Вивчення вхідних поверхневих доз пацієнтів при найпоширеніших рентгенографічних дослідженнях для встановлення національних діагностичних рекомендованих рівнів

Investigation of surface doses of the patients at common x-ray investigations with the purpose to establish national diagnostic recommended doses

Summary. The authors analyze the state of the park of x-ray diagnostic equipment, the structure of x-ray diagnostic investigations, frequency and conditions of x-ray diagnostic procedures using the findings of survey of radiology services of all regions of Ukraine in 2008. Entrance surface doses of the patients were measured according to IRPC recommendations and EU Manual for 10 common types of x-ray diagnostic investigations with the purpose to establish national diagnostic recommended levels. The findings of entrance surface doses of the patients are presented. Third quartiles were assessed for each type of investigation and compared with the values values of diagnostic reference levels in radiography according to Main International Standards of IAEA. The findings of the preliminary research demonstrate that in a number of x-ray diagnostic units patients exposure is ambiguously increased at x-ray investigations. Optimization of radiation protection of the patients at diagnostic exposure should be aimed at minimizing patients and personnel dose without deterioration of the obtained diagnostic information, which is possible with working out and introduction of the programs of quality assurance.

The experience of x-ray units control at treatment-and-prevention establishments and means of individual protection for correspondence to requirements of State Standard by an example of Kharkiv region

Summary. As it was determined by the control of the specialists of State Sanitary and Epidemiology Station of the region, Eastern State Inspection on Radiation Safety of Ukraine, 75% of x-ray equipment in Ukraine works longer than the warranted expiry date (10 years), therefore the control of dose-forming parameters is urgent. The performed work proved the necessity of taking out of operation of x-ray units which do not meet the requirements of State Standard and their replacement by modern digital low-dose equipment if modernization is not possible.

Assessment of radiation risk of mortality due to medical exposure

Summary. The risk of long-term negative effects is the main index of negative influence of patient medical exposure. The risk to the health due to medical exposure was calculated for the population of Zaporizhzhia region. It was established that the coefficient of possible death of the population in the region due to this cause makes 0.05-0.06 per one thousand, which is considerably lower than the minimal one according to the generally accepted scale.
