COMBINED ANALYSIS OF DYNAMIC HEPATOBILISCINTIGRAPHY AND SONOGRAPHY FINDINGS IN LIVER STEATOSIS

The analysis of anatomical and topographic as well as functional condition of the hepatobiliary system in liver steatosis was carried out due to ultrasound and radionuclide study methods. patients underwent sonography of the liver by means of ClearVue 650 Release 1.0 unit produced by «philips» through C5-2 transducer or M6-2 Active Array with the following intravenous dynamic hepatobiliary scintigraphy with 99mTc-Mezida, activity 1.1 MBq/kg. Owing to comparative analysis of the finfings, it was revealed that even in case of satisfactory sonographic presenation of the liver, DHBSG figures indicated detailed functional disorders. It causes a special concern due to the fact that the course of liver steatosis is not accompanied by evident clinical symptoms for a long time.

SYSTOLIC AND DIASTOLIC FUNCTIONS OF THE LEFT VENTRICLE OF THE HEART IN PATIENTS WITH CANCER IN DIFFERENT MODES OF CHEMOTHERAPEUTIC TREATMENT

The findings of echocardiography monitoring of patients who underwent treatment at National Cancer Institute were retrospectively analyzed. Echocardiography was performed in order to assess cardiotoxic impact of chemotherapeutic treatment. The use of echocardiography allowed revealing early, subclinical manifestations of cardiotoxicity, which is important when using chemotherapeutic schemes with cardiotoxicity.

SUBPOPULATIONS OF THYROCYTES IN RADIOIODINE-REFRACTORY METASTASES AND RADIOIODINE-UPTAKE METASTASES OF PAPILLARY THYROID CANCER

The cytomorphological study of fine-needle aspiration biopsy of 77 papillary carcinomas and 33 metastases made it possible to reveal heterogeneity of the population of follicular epithelial cells in radioiodine-refractory metastases compared to radioiodine-uptake metastases. Radioiodine-refractory metastases, unlike radioiodine-uptake metastases, contain several subclones of thyrocytes. Correlation between presence of special subpopulation of cells in primary papillary carcinomas and frequency of metastases after standard medical therapy (thyroidectomy, suppressive hormone therapy, radioiodine therapy) was revealed. It has been concluded that metastases appear more frequently in patients with particular subpopulation of tumor cells in punctates of primary papillary carcinoma than in patients without such cells.

METHOD OF VISUALIZATION OF CREBELLUM NUCLEI IN SPIRAL CT IMAGES IN STEREOTACTIC OPERATIONS

Abstract. Method of visualuzation of cerebellum nuclei in spiral CT images in stereotactic operations. For neurosurgical planning and neuronavigation tasks the method of spiral computer tomography (SCT) is used. Purpose. to visualize cerebellum nuclei in SCT images according to their stereotactic coordinates.

Material and methods. The study was performed on 430 specimens of the cerebellum of people aged from 20 to 99, 10 SCT images of the brain and cerebellum with the use of statistical analysis methods.

Results. In the course of the study the algorithm of multilane reconstruction of cerebellum nuclei on the axial tomographic slices of the cerebellum, which makes it possible to perform common surgical calculations without the use of traditional methods of contrast radiography, has been elaborated. Wherein the accuracy of reconstruction depends on scan step and it is of 1 mm order in higher information content associated with the principle of obtaining tomographic images.

In the programming language Borland Delphi v.7.0 using ApI OpenGL the software allowing to visualize the voxel model has been developed.

COMPARATIVE ANALYSIS OF BONE SCINTIGRAPHY IN PATIENTS WITH DEFORMING OSSTEOARTHROSIS AND AVASCULAR NECROSIS OF THE FEMORAL HEAD IN HIP ARTHROPLASTY

In order to carry out comparative analysis of bone scintigraphy findings during hip arthroplasty, the study enrolled 85 patients with deforming osteoarthrosis of hip and 65 patients with avascular necrosis of the femoral head.

Due to the study it was revealed that the quantitatively higher percentage of hyperfixation of radiopharmaceutical in patients with deforming osteoarthrosis is and, accordingly, the higher the percentage of hypofixation of indicator in patients with avascular necrosis before the endoprosthesis is, the significantly greater violation of the functional condition of the joint during the postoperative period (p <0,05) is.

Bone scan can be used in diagnostic screening of patients with deforming osteoarthrosis and avascular necrosis of the femoral head during hip arthroplasty, as well as to assess the timing of rehabilitation of patients in the postoperative period.

APPLICATION OF PERFUSION SPECT IN DIAGNOSIS OF CEREBRAL CHANGES IN PATIENTS WITH MILD BRAIN INJURY

The aim of the study was to estimate the possibility of application of perfusion SPECT in diagnosis of cerebral changes in patients with mild brain injury. The study group consisted of 36 patients with traumatic brain injury (TBI), 22 patients with clinical manifestations of moderate and severe traumatic brain injury. Identified pathological changes in the form of areas of focal reduction of cerebral perfusion were diagnosed in cortical and subcortical regions, including decreased perfusion in the homolateral hemisphere. These changes were similar and correlated to the data obtained due to computed tomography (CT).

In 14 patients with mild TBI, CT abnormalities were not observed, but owing to the findings of SpECT, changes in brain perfusion with multiple asymmetric areas of hypoperfusion in the strike zone were identified.

It has been established that SpECT is more informative in identifying the areas of pathological perfusion in comparison with CT in patients with mild traumatic brain injury. Brain SpECT makes it possible to determine pathological changes of cerebral tissue in the acute phase of traumatic brain injury when morphological ones are still not detected.

CLINICAL ASSESSMENT OF EFFECTIVENESS OF RADIONUCLIDE THERAPY WITH 153SM-OXABIFOR IN ONCOLOGIC PATIENTS WITH BONE METASTASES

Oncologic patients with bone metastases are considered as one of the most serious categories of patients who demand conducting effective and well-planned course of palliative treatment.

The aim of the research was to assess the effectiveness of radionuclide therapy (ROT) with 153Sm-oxabifor in oncologic patients with bone metastases.

Materials and methods. RNT was provided for 24 patients (18 women with breast cancer and 6 men with prostate cancer). 153Sm-oxabifor was introduced intravenously through an angiocatheter with the amount of activity of 11.03-90.82 MBq/kg. Scanning of the whole body was conducted in 3-72 hours after introducing 153Sm-oxabifor with the following assessment in comparison with the data of pre-therapeutic bone scans with 99mTc-pirfotechum. Results. After 17 (57 %) courses of palliative RNT steady decrease of the rate and duration of the pain syndrome 1-60 days after was registered. The duration of pain-free period varied from 3 to 10 months after the course of treatment. It was proved that patients who received the treatment doze of RPh with the treating activity of 37.37-55.5 MBq/kg developed analgetic end pint earlier (8.1 ± 1.3 days after the treatment) than the patients who received smaller amount of treating activity (p = 0.039, р = 0.004).

LOW RADIATION DOSES: SCIENTIFIC CONTROVERSY OR CONFRONTATION OF VIEWS

Thirty years have passed after Chernobyl Nuclear Power Plant accident, however there are still several doubts and concerns between international experts and authors of the various domestic publications dealing with the risk of low radiation doses, that requires well-planned study of any possible health impacts after the accident.

According to the domestic publications focused on the investigation of medical consequences resulting from Chornobyl Nuclear Power Plant accident, numerous systematic errors and outside factors are highly underestimated. This fact influences obtained findings.

Inaccuracy in the research planning, underestimation of systematic errors and outside factors will not make it possible to get new knowledge concerning the impact of low ionizing radiation doses on patients. The dataset dealing with of the total negative health impacts after the Chernobyl Nuclear Power Plant accident will be definitely obtained.

ROLE OF RADIONUCLIDE THERAPY IN ONCOLOGY

This article deals with clinical usage of radiopharmaceuticals in radionuclide therapy provided for oncological patients. Some examples of importance of radioiodine therapy for highly differentiated forms of thyroid cancer treatment and radionuclide-drug therapy schemes of mammary and prostate cancer with multiple skeleton metastasis have been suggested. Based on long-term experience in this field of oncology, the conclusion of importance and long-term perspective of radionuclide therapy in oncology, urology, endocrinology, hematology and other medical disciplines has been drawn.

UNDERGOING TREATMENT WITH EXTERNAL BEAM RADIATION THERAPY

The number of patients undergoing radiation therapy of prostate has significantly increased over the last decade. It results from rapidly growing technology used in radiation therapy which entails modernization of radiation therapy programs, new demands to quality assurance, fundamentally different approaches to treatment planning and target definition. These branches of radiation therapy development are particularly important due to topographic anatomy of organ localization and possibility of application of external beam and internal radiation therapy. It is obvious, that being developed in these hi-tech, knowledge intensive ways, radiation therapy of prostate cancer will be able to reach higher level and provide better local control at a low frequency of post treatment reactions.

MYELOID-DERIVED SUPPRESSOR CELLS AS A NEW THERAPEUTIC AIM IN ONCOLOGY

Myeloid-derived suppressor cells (MDSC) are considered as heterogeneous population of immature myeloid cells, mechanisms of action and clinical value of which are intensively studied over the last few years. The review deals with immunosupressive mechanisms of MDSC action as well as mechanisms, engaged in progression and metastasis of malignant tumours. The clinical findings focused on the analysis of prognostic value of MDSC in cancer patients have been summarized as well as different approaches aimed to decrease the ammount and/or functional activity of MDSC have been analyzed.
